Description

Boulder county rsvp board, INC. DBA as cultivate (formerly boulder county careconnect), was founded in 1972 with the mission to promote the security and independence of older adults and help individuals flourish through active engagement with their community. For over 46 years, cultivate has managed the boulder county rsvp program as part of our larger mission to serve seniors in need. Cultivate has always focused on aging in place, which makes the healthy futures: aging in place, the appropriate primary focus area for our rsvp program. The other rsvp stations in boulder county supplement cultivate's unique programs and services to provide even more support to adult adults aging in place in their communities. Continuing the rsvp program means at least 353 unduplicated rsvp volunteers will serve the community in 2019 and beyond at 23 stations. 250 rsvp volunteers will serve in outcome-based assignments. The outcome based activities in our primary focus area: healthy futures, are food delivery, transportation and housing services that are all critical needs facing our aging populations. The cultivate rsvp program also places emphasis on the focus area: veterans and military families by providing peer based medical transportation to VA hospitals and clinics. Throughout the three-year grant, at least 353 unduplicated rsvp volunteers will provide 1400 seniors with out-come based services that increase social ties, support independence, promote safety and help alleviate isolation. Cultivate surveys clients annually: 80% of individuals served will report that rsvp volunteers increased their level of social support. Of the 353 unduplicated rsvp volunteers, at least 228 will serve at cultivate stations and an additional 103 volunteers will serve the community in a variety of meaningful opportunities that address critical community needs including access to health care, education, disaster preparedness, environmental stewardship and other community needs, demonstrated in the other community priorities. The cncs federal investment of $80,694 for year 1 of the grant cycle will be matched by $43,135 by cultivate.
