EXECUTIVE SUMMARY SOUTHEASTERN VIRGINIA AREAWIDE MODEL PROGRAM, INC. DBA SENIOR SERVICES OF SOUTHEASTERN VIRGINIA (SSSEVA) IS THE AREA AGENCY ON AGING SERVING RESIDENTS IN THE CITIES OF CHESAPEAKE, FRANKLIN, NORFOLK, PORTSMOUTH, SUFFOLK, VIRGINIA BEACH AND ISLE OF WIGHT AND SOUTHAMPTON COUNTIES. SSSEVA DEVELOPS AND OPERATES COORDINATED PROGRAMS (MEALS, TRANSPORTATION, CARE COORDINATION, BENEFITS COUNSELING, ETC.) THAT ASSIST SENIORS 60 AND OLDER, THEIR FAMILIES AND CAREGIVERS. SSSEVA ALSO SERVES AS A RESOURCE CENTER FOR ADULTS AGES 60+ WITH DISABILITIES. OUR MISSION IS "TO PROVIDE SENIORS AND THEIR CAREGIVERS WITH ACCESS TO PROGRAMS AND SERVICES, SO THEY MAY LIVE THEIR LIVES WITH CHOICE AND DIGNITY IN THEIR COMMUNITIES." AS AN APPLICANT FOR THE SENIOR COMPANION PROGRAM (SCP), WE ARE REQUESTING 58 VOLUNTEER SERVICES YEARS (VSYS). TOTAL CNCS FUNDING FOR THE PROJECT IS $304,142. THE TOTAL PROJECT COST IS $337,936 WHICH INCLUDES THE 10% NON-FEDERAL SHARE OF $33,794. OUR REQUEST INCLUDES $4,000 OF THE AVAILABLE $15,071 IN ADDITIONAL FUNDING. WE WILL ALLOCATE THESE ADDITIONAL FUNDS TO THE SCP COORDINATOR'S SALARY, AS WELL AS PROVIDE FOR INCREASED VOLUNTEER SUPPORT. PLEASE SEE PROGRAM BUDGET SECTIONS FOR LINE ITEM DETAILS/JUSTIFICATIONS. SSSEVA WILL UTILIZE CNCS SCP GRANT FUNDS TO ADDRESS THE FOLLOWING SERVICE CATEGORIES WITHIN THE HEALTHY FUTURES FOCUS: AGING IN PLACE. ANTICIPATED OUTCOMES OF THIS PROJECT PER APPENDIX B: CORPORATION FOR NATIONAL AND COMMUNITY SERVICE SENIOR CORPS SENIOR COMPANION PROGRAM (SCP) NATIONAL PERFORMANCE MEASURES INSTRUCTIONS. PERFORMANCE MEASURES INSTRUCTIONS INCLUDE: CLIENTS: * NUMBER OF INDIVIDUALS WHO REPORT HAVING INCREASED SOCIAL SUPPORT OR IMPROVED CAPACITY FOR INDEPENDENT LIVING. INDIVIDUALS LIVE IN A PRIVATE RESIDENCE (HOME, APARTMENT, MOBILE HOME, ETC. RATHER THAN IN AN ASSISTED LIVING FACILITY, NURSING HOME, OR GROUP HOME). (AGING IN PLACE: OUTCOME - H9A) MEASUREMENT TOOL: SENIOR CORPS INDEPENDENT LIVING PERFORMANCE MEASURE SURVEY CAREGIVERS: * NUMBER OF CAREGIVERS OF OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES WHO REPORTED HAVING INCREASED SOCIAL SUPPORT. (INCLUDES RESPITE SERVICES THAT PROVIDE TEMPORARY RELIEF FROM THE STRESSES (AGING IN PLACE: OUTCOME - H14) MEASUREMENT TOOL: SENIOR CORPS RESPITE CARE PERFORMANCE MEASURE SURVEY SCP VOLUNTEERS WILL PROVIDE CLIENTS WITH COMPANIONSHIP AND ASSISTANCE WITH ACTIVITIES OF DAILY LIVING (ADLS) AND INSTRUMENTAL ACTIVITIES OF DAILY LIVING (IADL'S), ALLOWING THEM TO LIVE MORE INDEPENDENTLY AND PROVIDING FOR CAREGIVER RESPITE. SCP VOLUNTEERS VISIT CLIENTS ONE TO FIVE DAYS PER WEEK FOR A MINIMUM OF FOUR HOURS PER DAY OVER 52 WEEKS PER YEAR. SENIOR SERVICES CURRENTLY SPONSORS 14 VOLUNTEER STATIONS THROUGHOUT OUR EIGHT SERVICE JURISDICTIONS. WE DIRECTLY OPERATE EIGHT (8) OF THESE VOLUNTEER STATIONS WITHIN OUR LOCAL OFFICES AND/OR WELLNESS CENTERS, AND PARTNER WITH AN ADDITIONAL SIX (6) COMMUNITY PROVIDERS, HEALTH DEPARTMENTS, DEPARTMENTS OF SOCIAL SERVICES, AND HOUSING AUTHORITIES THAT SERVE OUR TARGET POPULATION OF SENIORS AND ADULTS WITH DISABILITIES 60 YEARS AND OLDER. OUR VOLUNTEER STATION ROSTER HAS CHANGED SINCE LAST YEAR'S CONTINUATION GRANT SUBMISSION. WE WERE UNABLE TO MAKE A SCP VOLUNTEER MATCH AT JEWISH FAMILY SERVICES - EVEN AFTER MULTIPLE ATTEMPTS. THIS STATION IS CURRENTLY INACTIVE. WE ARE WORKING TO ESTABLISH VOLUNTEER STATIONS THROUGHOUT OUR SERVICE AREA AND TO EXPAND PARTNERSHIPS OUTSIDE OF THOSE WE NORMALLY CULTIVATE. FOR INSTANCE, OUR OBJECTIVE IS TO MAINTAIN CURRENT COLLABORATIONS AMONG OUR TRADITIONAL PARTNERS WHILE CULTIVATING RELATIONSHIPS WITH PRIVATE SENIOR HOUSING COMPLEXES (THOSE NOT OPERATED BY LOCAL HOUSING AUTHORITIES) AND LOCAL CHURCHES IN LOW-INCOME NEIGHBORHOODS. OUR GOAL IS TO INCREASE OUR VOLUNTEER STATIONS FROM 14 TO 17 OVER THE NEXT THREE YEARS. THESE PARTNERSHIPS ARE CRITICAL TO PROGRAM DELIVERY TO CLIENTS.
