THE PRIMARY FOCUS AREA OF THIS PROJECT IS HEALTHY FUTURES, AGING IN PLACE. AT THE END OF THE THREE-YEAR GRANT: * INDEPENDENT LIVING SURVEYS WILL SHOW 80% OF THE 150 OF HOMEBOUND SCP RECIPIENTS REPORTED HOMEBOUND OR OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES WHO REPORTED HAVING INCREASED SOCIAL TIES/PERCEIVED SOCIAL SUPPORT. * RESPITE CARE SURVEYS WILL SHOW THAT 80% OF 35 CAREGIVERS OF HOMEBOUND OR OLDER ADULTS AND INDIVIDUALS WITH DISABILITIES WHO REPORTED HAVING INCREASED SOCIAL TIES/PERCEIVED SOCIAL SUPPORT. THE CNCS FEDERAL INVESTMENT OF $342,575 WILL BE SUPPLEMENTED BY $106,470 OF NON-FEDERAL RESOURCES.
