Description
Our mission is dual: to immediately improve the health of communities and vitality of primary care practices while exposing tomorrow's most promising future physicians to the great potential for community impact possible through primary care practice in "medically underserved" communities. Our proposed vista project aligns with americorps vista's healthy futures programming priority area, in that IT supports improved primary care access in communities of poverty in north carolina. This application seeks 4 vistas to support capacity-building endeavors expanding medserve connections with community clinics; and spearheading improvement processes and resources for medserve and stakeholders to better serve low-income communities. Specifically, these resources will provide tangible assets whereby primary care clinics can better utilize direct service participants to care for more patients in low-income communities. We expect to benefit approximately 40 primary care clinics and 30,000 community members treated at those clinics per year. The project will run in 1 year increments over a 3-year period that aligns with the americorps grant program. At the conclusion of this program year, we expect to be able to transition all vista capacity-building projects back to staff.
