Age Well serves nearly 10,000 older Vermonters annually across Addison, Chittenden, Franklin, and Grand Isle Counties.
This is a formula grant awarded on a non-competitive basis upon approval of the State Plan. The program supports evidence-based disease prevention and health promotion services including physical fitness, medication management, chronic disease self-management education, psychosocial behavioral health intervention, falls prevention, brain health, diabetes management, arthritis, depression, chronic pain, and HIV services.
