Description

North dakota department of corrections and rehabilitation will implement a peer support reentry project to improve continuity of care for adults with substance use disorders, mental health disorders, and CO-occurring disorders leaving correctional facilities. The project will serve individuals transitioning from incarceration to the community, with priority for those who have participated in residential treatment or are otherwise at high risk for relapse and destabilization. A certified peer support specialist will provide individualized recovery planning, crisis response, referrals, and ongoing linkage to treatment, housing, healthcare, transportation, and employment resources. The project will also coordinate with community providers to support timely access to aftercare services and strengthen recovery during reentry. Expected outcomes include increased engagement in services, improved stabilization, better recovery goal attainment, and stronger connections to community-based support systems.
