In partnership with the Idaho Community Health Center Association, Comagine Health will implement a comprehensive, evidence-based initiative to increase colorectal cancer screening rates among rural Idahoans aged 45-75 receiving care at Federally Qualified Health Centers. The project targets five FQHCs collectively serving 121,519 patients with screening rates below the national average and will operate through a five-year performance period concluding August 29, 2030. The cooperative agreement funds a phased, cohort-based service delivery model employing two successive cohorts of FQHCs over 2.5 years each. Core services include collaborative outreach and readiness assessments to identify and recruit participating FQHCs while addressing structural barriers such as transportation and financial assistance; facilitation of quarterly learning collaborative sessions and monthly technical assistance using the IHI Breakthrough Series Model to implement evidence-based interventions including patient reminders, provider reminders, provider assessment and feedback, and patient navigation services; and comprehensive monitoring and evaluation activities utilizing the Azara Population Health System to track screening and follow-up data with quarterly performance reporting.
