SOME OF OUR DESIRED OUTCOMES ARE TO IDENTIFY SUID/SDY CASES IN A CONSISTENTLY TIMELY MANNER, ENTER DATA INTO THE NFR-CRS IN A MORE COMPLETE AND TIMELY MANNER, PERFORM QUALITY CHECKS ON ALL OUR SUID/SDY DATA, AND DISSEMINATE OUR DATA TO INTERNAL AND EXTERNAL PARTNERS. THE ULTIMATE GOAL OF THIS PROJECT IS TO INCREASE THE NUMBER OF DATA-DRIVEN, INFORMED PRACTICE AND POLICY CHANGES, WHICH IN TURN WILL HOPEFULLY DECREASE SUID/SDY RATES AND REDUCE DISPARITIES. THIS APPLICATION BUILDS UPON THE EXPERIENCE AND LESSONS LEARNED FROM PARTICIPATING IN THE LAST 5-YEAR CYCLE OF THE SUID AND SDY CASE REGISTRY.
