Description

Improving the health of people with mobility limitations and intellectual/developmental in michigan - the disability health program (DHP) at the michigan department of health and human services (mdhhs) has been funded by the CDC since 2007 to improve the health of people with disabilities (PWD), and improve the accessibility of the state?s public health programs.DHP has built and sustained a coordinated public health approach to improving the health of PWD through strong data collection, improving access to public health programs, instituting policy change, educating healthcare and public health professionals in access and inclusion, and implementing evidence based health promotion programs. This proposal provides an opportunity to expand these efforts using the following strategies.1. Expand partnerships. Our existing advisory council (AC) includes disability advocacy and service organizations, public health and implementation partners. We will expand membership to include partners whose work complements the strategies in this application, including more PWD and families, health systems, recreation and community organizations.2. Conduct needs assessments. DHP will work with the developmental disabilities institute (DDI - our state?s university center of excellence in developmental disability) to conduct a statewide needs assessment for people with intellectual/developmental disability (IDD) and their families, identifying gaps in resources/services and barriers to utilization, and, in partnership with our AC, create a plan to address these needs.3. Train healthcare providers. In collaboration with DDI and other grantees, DHP will identify existing disability competency training for healthcare providers, and use their extensive provider networks to recruit participants. DHP and DDI staff will deliver the selected trainings and collect data on effectiveness and reach.4. Implement and evaluate a demonstration project; create community resource directories. Working with DDI and other awardees, DHP will implement a demonstration project to increase healthcare screening and rates of preventive care among people with IDD in the detroit metro area (the most populous area in the state, and the most affected by disability and health disparities), recruiting and retaining at least 200 people with IDD during the funding period. DHP will also work with the national kidney foundation of michigan (nkfm) in the creation of an accessible, plain-language guide to locating and accessing resources for health and social needs, starting with metro detroit and adding a new area of the state each grant year.5. Implement evidence based programs (ebps) and policy/systems change. DHP and nkfm have significant experience in accessibly-delivered ebps for people with IDD and mobility limitations (ML). We have networks in place to deliver a variety of health promotion or chronic disease management programs: walk with ease (an educational walking program for people with ML); stanford?s chronic disease self-management program; enhancefitness (an inclusive exercise program for PWD and older adults, including those with IDD); and/or the diabetes prevention program for people with IDD. Should the grantees select a different EBP, the DHP and nkfm have capacity to train instructors and implement the requested program. DHP will effect policy change by training our bureau?s diversity hiring team in accessible recruiting and hiring, ensuring disability competency on all hiring teams; partnering with the michigan parks association to seek changes in the accessibility of select parks through the installation of power wheelchair charging stations; and working with the healthy dearborn coalition to include disability initiatives in community programming.6. Disseminate key findings. DHP will collect lessons learned from the above activities and share with public health, healthcare, disability and government partners over the life of the grant.DHP will implement our eval
