Description
Diabetes prevention and treatment project - based on the canoncito band of navajos health center (CBNHC) database there are 227 patients with diabetes mellitus type 2 and 281 patients with pre-diabetes empaneled to our health center. Blood pressure in control rates for our patients with diabetes mellitus type 2 has been low at 54% and 52% over the last 2 years according to our sdpi diabetes audit reports. Blood pressure control reduces the risk for complications from hypertension and diabetes such as cardiovascular disease and kidney disease. CBNHC has selected improving blood pressure in control rates for patients with diabetes who live in the community of to'hajiilee, NM as the sdpi best practice. Within the health center we plan to provide education to clinical providers on the importance of controlling blood pressure with evidence based treatment including lifestyle changes as well as medication management. Community outreach will involve providing home blood pressure monitors and planning activities and challenges that encourage exercise, healthy low salt eating and weight loss. Our second best practice will be to continue to improve DM eye care by scheduling annual eye exams and providing prevention education of eye care. Our third best practice will be to continue to improve diabetes foot care. We will hire a podiatrist for foot care and we will provide outreach prevention education on the importance of foot care and good DM control.
