Description
Strategies to deliver & sustain the well-being action network of NH - new hampshire (NH) proposes to expand and sustain its well-being action network (wban) which offers self-management programs for chronic disease, chronic pain, diabetes, and walk with ease (WWE). Since 2010, NH has maintained a statewide network of sites and leaders offering cdsmp. NH has 1.4 million people and an aging population. Our targeted impacts include seniors and individuals with disabilities, prioritizing those with behavioral health conditions such as depression, anxiety, or substance use disorders as well as those experiencing chronic pain or stressors that negatively impact behavioral health, such as a new diagnosis. Goals are to: (1) develop capacity to increase the number of older adults and adults with disabilities who participate in chronic disease self-management education by 600 participants and 548 WWE participants over three years, and (2) develop and disseminate 508-compliant resources to enhance capacity to serve the target population and aid in the sustainability of programs. NH partners with the bureau of aging & adult services, community behavioral health association, UNH extension, and other partners to increase program referrals, sites, and leaders. Partners ensure statewide engagement through increased outreach, and capacity building. UNH institute on disability will provide technical assistance to develop 508-compliant resources such as brochures, forms, slide decks, and websites that will result in a capstone project highlighting lessons learned to support replication by others in the field.
