About this award

The Department of Health and Human Services Administration for Community Living awarded Mid-America Regional Council Community Services Corporation a $1.199 million cooperative agreement on August 1, 2026, to expand chronic disease self-management education (CDSME) and behavioral health integration across Missouri's Community Integrated Health Network.

The recipient will deliver evidence-based CDSME programs to 1,333 participants across nine program models—including Chronic Disease Self-Management Program, Disease Self-Management Program, Chronic Pain Self-Management Program, Wellness and Chronic Disease Self-Management Program, Tomando Control de Su Salud, Cancer Surviving and Thriving, Walk with Ease, EnhanceFitness, and Healthy Ideas—with a target completion rate of 1,000 participants and 40 percent reporting improved self-efficacy in managing chronic conditions. The award supports screening 500 adults for behavioral health needs including depression and anxiety; expanding the network to include behavioral health providers through eight bidirectional agreements; training 50 CDSME leaders; improving data usage and reporting capabilities; and securing two new CDSME funding sources. The network comprises ten area agencies on aging, 35-plus service providers, and six behavioral health providers, with statewide partner organizations including the Missouri Council on Aging and Missouri Veterans Commission.

The award funds work in Missouri through July 31, 2029, under the Empowering Older Adults and Adults with Disabilities through Chronic Disease Self-Management Education Programs (CFDA 93.734), financed by Prevention and Public Health Funds. The program supports increasing CDSME participation while enhancing program sustainability within aging and disability networks.
