Education Health & Research International Inc. The award funds the "EHRI CDSME Behavioral Health" initiative, which delivers evidence-based Chronic Disease Self-Management Education (CDSME) programs to Delaware seniors and disabled adults aged 60 and older experiencing behavioral health challenges, including depression, anxiety, post-traumatic stress disorder, and acute behavioral health stressors such as grief, financial changes, or new chronic diagnoses. EHRI will accomplish this mission by expanding statewide CDSME delivery capacity through recruitment and training of 40 new lay leaders (doubling an existing network of 43 volunteers), developing both in-person and virtual program delivery options across multiple sites and formats, and recruiting program participants throughout Delaware. The initiative leverages a network of eight partner organizations—including the Mental Health Association in Delaware, Jewish Family Services, LARED (a Federally Qualified Health Center), Beebe Healthcare, Modern Maturity Center, Pace Your Life, EasterSeals, and Reliant Care Solutions—to ensure program sustainability and replication. Additionally, EHRI will integrate CDSME behavioral health services with its statewide WeCare nurse-based care navigation network to provide wraparound support services to participants.Summary
