HQP FY22 CONNECTING KIDS TO COVERAGE - HEALTH QUALITY PARTNERS (HQP) AND SIX HEALTH CENTER ORGANIZATIONS LOCATED IN SAN DIEGO AND RIVERSIDE COUNTIES IN SOUTHERN CALIFORNIA WILL PARTNER TO IMPLEMENT THE FY22 CONNECTING KIDS TO COVERAGE (CKC) COOPERATIVE AGREEMENT TO INCREASE ENROLLMENT OF ELIGIBLE CHILDREN INTO MEDICAID (ALSO KNOWN AS MEDI-CAL IN CALIFORNIA). AS THE LEAD APPLICANT, HQP WILL SUBCONTRACT WITH SIX FQHC COMMUNITY HEALTH CENTER ORGANIZATIONS THAT WILL EMPLOY CERTIFIED ENROLLMENT COUNSELORS (CEC) TO IDENTIFY ELIGIBLE CHILDREN, PROVIDE ONE-ON-ONE ENROLLMENT SERVICES, VERIFY ENROLLMENT, AND CONDUCT ANNUAL RENEWAL FOLLOW-UP TO ENSURE RETENTION. IT IS ANTICIPATED THAT 15,000 CHILDREN WILL SUBMIT AN APPLICATION FOR HEALTH COVERAGE, 4,080 WILL BE NEWLY ENROLLED, AND 7,920 SUCCESSFULLY RETAINED OVER THE THREE-YEAR PERIOD. HQP WAS A GRANTEE OF CMS DURING CYCLE IV OF THE CONNECTING KIDS TO COVERAGE PROGRAM AND IS A CURRENT GRANTEE OF THE FY19 CKC PROJECT. ALL SIX PARTICIPATING HEALTH CENTERS SERVED AS SUBCONTRACTORS OF HQP DURING THE CYCLE IV AND FY19 PROJECTS AND ARE OPERATIONALLY READY TO CONTINUE PROVIDING SERVICES TO CHILDREN AND FAMILIES IN THE NEW THREE-YEAR PROJECT. PROJECT PARTNERS WILL IMPLEMENT CLINIC INREACH AND COMMUNITY OUTREACH STRATEGIES THAT HAVE PROVEN SUCCESSFUL DURING THE PREVIOUS TWO CYCLES TO ENROLL CHILDREN WHO REMAIN UNINSURED. HQP WILL PUT SPECIAL EMPHASIS ON VERIFYING ENROLLMENT TO ENSURE THAT A MINIMUM OF 80% OF APPLICANTS (BOTH NEW ENROLLMENTS AND RENEWALS) ARE PROCESSED SUCCESSFULLY THROUGH TO VERIFIED ENROLLMENT. PROJECT PARTNERS WILL ALSO PARTICIPATE IN CMS NATIONAL CAMPAIGN ACTIVITIES TO INCREASE ENROLLMENT IN MEDICAID AND PARTICIPATE IN TARGETED OUTREACH AND EDUCATION TO INFORM FAMILIES WITH CHILDREN OF THE NEED TO RENEW ENROLLMENT AS THE COVID-19 PUBLIC HEALTH EMERGENCY ENDS. THROUGH COORDINATION WITH THE COUNTY OF SAN DIEGO AND LOCAL OUTREACH AND ELIGIBILITY ADVOCATES, HQP WILL PROVIDE CECS WITH REGULAR TRAINING AND NETWORKING OPPORTUNITIES TO IMPRO VE PERFORMANCE AND ENCOURAGE THE SHARING OF BEST PRACTICES. THE THREE-YEAR BUDGET REQUEST FOR THIS PROGRAM IS $1,500,000.
