Added: Sep 06, 2011 9:17 pm This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; a written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This combined synopsis/solicitation SHALL be posted on both FEDBIZOPPS and NECO (http://www.neco.navy.mil/). The RFQ number is N00604-11-T-3218. This solicitation documents and incorporates provisions and clauses in effect tough FAC 2005-53 and DFARS Change Notice 20110518. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses: www.acquisition.gov/far and http://www.acq.osd.mil/dpap/dfars/index.htm. The NAICS code is 336211 and the Small Business Standard is 500. The proposed contract is 100% set aside for small business concerns. BRAND NAME ONLY: The proposed contract action is for a brand name only. The brand name and model number of the product(s) are: Maxon Lift Gate Model GPTLR-44. In accordance with FAR 5.102(a)(6), the required justification or documentation is attached. The NAVSUP Fleet Logistics Center Pearl Harbor requests responses from qualified small business sources capable of providing the following brand name items: CLIN 0001 – FURNISH AND INSTALL MAXON LIFT GATE GPTLR-44 (68" X 80") - FOR TRUCKS 95-31611 & 96-45911, DUAL STEP, PRE-FIT & ALIGN LIFT GATE. INSTALL PUMP, HYDRAULIC LINES, ELECTRICAL FROM BATTERIES TO LIFT GATE. WELD LIFTGATE, REFILL HYDRAULIC FLUIDS, BLEED FLUIDS AND TEST LIFT GATE. DISPOSE OF HAZMAT & DAMAGED PARTS. FREIGHT CHARGES INCLUDED. 2 YEAR WARRANTY FOR MAXON LIFTS. QTY 2 Ea @ $_____ = _¬____. Delivery Date: 30 NOV 2011 - If this date cannot be met, contractors must provide an accurate and realistic delivery schedule. Evaluation Criteria: Procedures in FAR 13.106 are applicable to this procurement. While price will be a significant factor in the evaluation of offers, the final contract award will be based on a combination of factors - a combination of technical acceptability, delivery, and price.
