Added: Nov 04, 2014 5:01 pm This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 - streamlined procedures for commercial items, in conjunction with FAR Part 13 - Simplified Acquisition Procedures; and as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect tough the Federal Acquisition Circular 2005-74, dated November 1, 2014. Solicitation VA262-14-Q-0028 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm Fixed Price award with a base year and up to four (4) option years. This procurement will be issued under an unrestricted basis. NAICS Code is 811211 and size standard is $7.5 million. The Government is seeking a turn-key preventive and corrective maintenance service contract for various models of Government-Owned Level 1, Hotline Blood/Fluid Warmers, in accordance with OEM, Smiths-Medical Asd, Inc. specifications and all terms, conditions, special provisions, statement of work, and schedules herein. There are a total of thirty-tee (33) blood/fluid warmers located at VA Greater Los Angeles. Please see attached inventory sheet for the location of each individual models and warmers. The Turn-key solution shall be inclusive of all necessary labor, material, parts, transportation, equipment, and supervision to perform the requested service. Place of performance: VA Greater Los Angeles Healthcare System, 11301 Wilshire Blvd, Los Angeles, CA 90073. See attached SF1449 Solicitation for statement of work (SOW) and other pertinent information. Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on a Pass/Fail evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government. Submitting Quote: Offeror(s) shall submit their quote on company letterhead and shall include the completed pricing forms, along with name, address, telephone number of the offeror, firm's DUNS#, total price and a completed copy of Clause 52.212-3 Offerors Representations and Certifications- Commercial Items or vendor's ORCA document. As an alternative, Offeror(s) may use the Price/Cost Schedule of the attached SF 1449 for quote submission. Submit all question(s) to Layton Matthews at Layton.matthews@va.gov no later than November 6, 2014, 2:00 PM Pacific Standard Time (PST). Ensure to reference solicitation # VA262-14-Q-0028 within subject line of the email. All questions and answers will be posted to this solicitation via an amendment no later than November 7, 2014 3:00 PM PST. Quotes must be received November 10, 2014 by 3:00 PM PST.
