STATEMENT OF WORK

Magnitude of Construction: $250,000.00 - $500,000.00

West Cardio Room 121

Remove and Replace existing louvers with new awning windows. Awning window shall have aluminum framing and shall be the same locking mechanism as the weight room.

Remove the entire flooring and replace with a (monolithic), dual-durometer, padded polyurethane athletic flooring system. SIGNAFlor or approved equal. Floor is approximately 1681 SF.

Replace 8 each existing duplex receptacles along the walls and add 6 each new receptacles. Use wiremolds for these new receptacles. See electrical layout.

Install new 20 amps dedicated circuit duplex receptacle recessed on the floor. Receptacle shall be GFCI, have cover that can be close when plug is inserted to the receptacle, and flush with the finished floor. See electrical layout.

Remove and replace 8 feet x 4 feet mirror with wooden frame. New mirror shall be the same size with metal trim.

Repaint the entire interior with latex, semi-gloss, the same color including trusses. Exterior surfaces of new CMU shall be prepare, prime, and paint with 2 coats of the same color and scheme as the existing.

Install large ceiling fan (BigAss 10’ POWERFOIL X 3.0) or approved equal. Controller shall comply with NEC and UL standards, and shall be labeled as required by code. See fan layout.

STATEMENT OF WORK

Red machine needs dedicated circuit receptacle.

Orange machine needs 20 amps receptacle.

Moving, reinstalling, and purchasing of equipment is not included in the SOW.

The rest of the opening shall be block off with split face CMU.

East Cardio Room 118

Replace all louvers with aluminum frame awning windows on the east side of the room. Remove all louvers and plywood where AC located and block the area with drywall inside and EIFS siding outside side.

2. Remove the entire flooring to include raised floor and replace with a (monolithic), dual-durometer, padded polyurethane athletic flooring system. SIGNAFlor or approved equal. Floor is approximately 1225 SF.

Replace 9 each existing duplex receptacles along the walls and add 1 each new receptacles. See electrical layout.

Install 10 each new 20 amps dedicated circuit duplex receptacle recessed on the floor. Receptacle shall be GFCI have cover that can be close when plug is inserted to the receptacle, flush with the finished floor and equally space between walls. See electrical layout.

Remove all existing TV monitors receptacles. Install 4 each new 20 amps duplex receptacle for a new TV monitors to the adjacent wall. Receptacle shall be 3 feet apart at center of the wall and the same height as the existing

6. Replace 1 each 10 sectional 20 feet x 8 feet wall mirrors with 5 sectional 20 feet x 4 feet wall mirror. Replace 2 each 4 feet x 8 feet wall mirror with 4 each 4 feet x 4 feet. Replace 1 each 5 feet x 5 feet with 4 feet x 4 feet. New mirror shall have metal trim. Mirrors shall be installed in the same location but above the wainscot line.

Repaint the entire interior with latex, semi-gloss, the same color. New exterior surfaces shall be prepare, prime, and paint with 2 coats with the same scheme and color as the existing.

8. Replace all drop ceiling tiles approximately 1225 SF. Ceiling grids and Light fixtures to remain.

Remove and replace 3 each 30,000 BTU split AC system to include piping, wirings and 30 amps disconnects.

New units shall be 36,000 BTU Model MITSUBISHI MUY-D36NA-1 for outside units and MSY-D36NA-8 for the inside units or approved equal. The same circuit to be use. New disconnect shall be NEMA 4X. All insulated piping exposed to the weather shall have an aluminum or PVC jacket. See drawing.

STATEMENT OF WORK

Court Area

Remove existing water fountain immediately outside of weight room and replace with two each side by side. Water fountains shall have automatic water dispenser. See picture below.

Install 3 concrete pads (outside the women's locker room, east of the RB court #9, and west of RB court #1) approximately 765 SF. Concrete pad shall be 4 inches thick, broom finish, 3,000 psi, ¾ aggregate, reinforced with 10/10 welded wire mesh.

Replace 6 inches wide interior courtyard drain covers approximately 520 feet long. Drain cover shall be 6 inches by 24 inches sections and can stand traffic load of 326-575 psi. Specification is NDS DS-232, 2 ft. Ductile Iron Channel Grate, Black, ADA or approved equal.

Install 5 each Lumecon LED flood lights, yoke mounted, 80-120W range. Light shall be installed on the top edge of the Gym overhang. Power supply from weight room panel and switch beside the panel.

New Water Fountain

Install 9’ x 15’ concrete pad.

Install 12’ x 23’ concrete pad Install 11’ x 32’ concrete pad.

Install 5 each LED flood lights

Panel in Weight Room Cutler Hammer, PRL1a Panelboard, 3 P, 225 A, 208Y/120 V.

Flood Lights Power Source

NOTE:

1. The entire installation shall comply with all applicable requirements of the current versions of the International Building Code, Plumbing Code, National Fire Protection Association, NFPA 90A, ASME Pressure Piping Code, Federal OSHA codes, National Electrical Code, DPW AC guide lines and all other agencies having jurisdiction.

2. The Contractor shall verify all measurements, equipment sizes, weights, connection locations and required clearances for installation and maintenance at the jobsite and coordinate their requirements with the manufacturer. All equipment supports and piping shall not obstruct equipment access areas for maintenance service and repairs.

Repair any damage to existing construction resulting from the installation of window and mechanical items. The areas repaired shall match the adjacent surfaces in texture and color.

All new equipment shall be labeled with the same nomenclature as the replaced equipment. Labels shall follow ASME

(ANSI) Standard A13.1-2007 for label color, text color, and label size and text.

The SOW is intended to cover the complete installation of systems to function as described and specified. The omission of reference to any necessary item of labor or material shall not relieve the contractor from providing such labor and material.

7, All equipment must be capable of fitting into the spaces allocated while meeting the manufacturer's recommended access requirements. Review all spaces where equipment is to be installed prior to ordering of equipment and notify the project manager of any inadequate clearances or conditions that will prevent the proper installation, maintenance and operation of the equipment.

8. Properly firestop all penetrations through fire rated walls, floors, or partitions with a UL approved system appropriate for the penetration type and fire rating.

Contractor shall dispose all recyclable materials according to DPW C&D waste guide line. Form attached.

Contractor shall submit a project specific AHA. Contractor to provide pertinent certifications that were not submitted in the Master Safety Plan & AHA and also indicate which certifications were included in the Master Safety Plan & AHA.

11. Contractor shall submit AC Unit Specifications, Awning windows, and flooring specification and color selection chart.
