GENERAL QUESTIONS

1. What is the project duration for the seed project?

250 Calendar days

2. For the seed project, what award/start date should we assume for the work schedule?

250 calendar days after NTP

3. How long will each of the rooms be allowed to be closed for the renovation to be completed?

To be coordinated by the user; however, 250 after the NTP is issued.

4. How far in advance does the contractor need to notify the facility of a utility shut down?

At least 2 week minimum

5. How many calendar days will the project have to completion from the NTP date?

250 calendar days after NTP

6. Will the contractor be allowed to work in multiple project rooms/areas at a time during the project or only one project room/area at a time?

Yes, however rooms/areas to be coordinated with user.

7. Is there a scheduled time frame or duration for the seed project?

8. Is there a government estimate cost range for this project?

Magnitude of Construction is $250K to $500K

10. Performing this work will require closing various rooms/sections of Building 488 – is the projects to be done in stages?

Yes We suggest that working in stages will work best for End Users of the facility and for the stated goals of this MATOC contract. Specifically, if the goal is to provide field worker opportunities to women and minorities – a staged project – utilizing smaller crews – will increase the percentages of women and minorities.

11. What are the days and hours that the contractor will be allowed to work?

Refer to Para 8 of General Information. 7:30AM to 4:30PM. Monday thru Friday

12. Is there any asbestos in the chilled water piping insulation?

There is no chilled water piping insulation.

13. If the seed project does not have test reports for Asbestos will the cost of testing need to be in our price?

Asbestos (ASB) and Lead Paint (LBP) lab analysis is attached.

14. If the Seed project test positive for Asbestos will this be negotiated as a change order to abate after award?

There is no asbestos.

15. Has a hazmat report for lead and asbestos been completed for this project? If not, how should we price for unknown hazardous materials?

Yes. ASB and LBP lab analysis is attached.

16. On the grates in the common area, do you want these to flat, or con‐caved like the existing?

The new cover shall be NDS #DS-232 2 ft. Ductile Iron Channel Grate or approved equal. See revised SOW and attached specification.

17. Is there a requirement for type of anchors or hangers that will be used for this project such as stainless, galvanize and etc?

No.

18. Please provide existing as-built architectural and electrical drawings for the areas that this project will cover?

As-built drawing attached. Existing electrical drawing on file has no significant use to this project. **This may not be the latest as-builts version. Contractor to field verify.

19. Are there any as-builts of the building that can be provided?

As-built that we see has significant to the project are attached. **This may not be the latest as-builts version. Contractor to field verify.

20. Will spraying painting be allowed?

No.

21. There were a lot of people at the site visit and it was difficult to look at the project site or take measurements. Please consider scheduling another site visit for the seed project.

22. Will the Statement of Work be revised per site visit discussions/announcements?

Yes, see revised SOW attached.

23. We are attempting to put the floor plan sketch of the Fitness Center into CAD so we can do a more precise estimate. Is it possible to get the floor plan that was in the PowerPoint (page 12 of scope of work) as a PDF?

As-built attached. **This may not be the latest as-builts version. Contractor to field verify.

FLOORING

24. Please provide a detail sheet on flooring system. Poured (monolithic)? Is it intended to saw-cut concrete to install floor boxes? Please provide additional info on panels, conduit routes, and locations.

Concrete floor shall be cut to install conduits and boxes. See revised SOW.

25. What is the desired thickness of the new flooring? The specified manufacturer offers this type of flooring in system-thicknesses of 9mm (standard) as well as 6mm, 8mm, 11mm, 15mm, and 17mm.

27. Is there a spec for the new flooring to be installed?

SOW cites SignaFlor or approved equal.

28. Is the raised floor to be removed a concrete pad?

No.

29. Please clarify if the amount of concrete pads has been reduced and locations.

Regarding the flooring / mastic. Do we assume HAZMAT (abatement) conditions? Or should we assume for testing with a possibility of Abatement?

31. Can the Government provide the test reports for the tile (VCT) or confirm there is no Asbestos concerning the seed project.

WINDOW

32. Is there a window spec and window schedule?

No window specification. Follow new windows style at the weight room. Supplier

(Arcadia or approved equal) information attached for reference only.

33. Need to know total number of windows, color/finish, desired glass make-up, rating -

ATFP, Blast, etc.

Total awning windows is 69. (Room 118 – 9 awning windows see page 10 of SOW &

Room 121 – 60 awning windows see page 5 of SOW). Contractor to field verify. Follow window at the weight room. See attached window specification. (Arcadia or approved equal) No ATFP or Blast Proof, etc.

34. In the site visit, it was stated that the specifications for the awning windows would be provided. When can we expect these specifications?

35. Since it was mentioned to match the windows of the gym area ‐ can we get who supplied them?

36. Awning windows shall have the same locking mechanism as the weight room ‐ does this mean that you want the individual lock on each awning leaf in the middle of the window and not a crank system to open all at once?

37. Are the new awning windows to be storm or hurricane rated? Do we need to comply with Anti-Terrorism requirements?

38. Do you have the manufacturer of the aluminum awning frame windows installed on the Gym?

40. Page 5 of the scope of work – SOW - PowerPoint document highlighted one set of the louvers in blue stating – “Remove louvers” – please confirm that how many sets of louvers

- in both photos – are being removed. Is it all three sets?

All louvers are to be removed where new awning window are to be installed.

ELECTRICAL

41. What is the specification for the new electrical conduit that the contractor will be using for the new circuits that are being required? ie: Electrical Molded Tubing (EMT) or Rigid.

Use Wiremolds on the walls. Follow NEC on dedicated outlets. See SOW notes.

42. Does all new electrical conduit need to be painted? If so please provide the color and type of paint that will be required?

Yes, paint to match existing area. Semi-Gloss/Latex. Color to be addressed at Precon.

43. Please clarify the exterior split system units are to be relocated as discussed onsite, if so please provide location of the new units.

44. Please provide the manufacturer information or pictures of the panels in order to provide new circuit breakers.

45. Page 5 of the SOW states that the area below + 36” – under the new Awning Windows -will be blocked off with CMU. The existing CMU on the building is split face (see photo below) is this the type of CMU desired in this location?

46. Electrical General – Can you please provide the as built electrical drawings for building 488?

Electrical as-builts are not available.

47. Electrical Recessed Outlets – Do you have a spec on this? Most outlets of this type are very deep.

48. Electrical LED Lighting – We noticed that there is an “Exterior & Night Light Control” Panel with contactors in the Electrical Room. Would you like the LED to be controlled from here? The panels in the gymnasium may not be as well suited for the LEDs.

New LED lights shall be manually controlled. Power supply from weight room panels.

49. After review of the electrical panels there is no capacity for the new dedicated circuits and new exterior lighting. Please confirm where/how we should provide power for the new electrical work.

ROOM 118

50. Please provide lead and asbestos reports for Cardio Rms 121 and 118.

51. Can you provide specs for the 2x4 acoustical ceiling to be installed in Rm 118 ?

52. What is the minimum thickness of the glass mirrors in Rm 118 ?

1/4” minimum thickness.

53. SOW for Rm 118: Item 7. Repaint the entire "exterior", (should be interior)?

55. Can you please confirm if we can work in Room 118 and Room 121 at the same time or do we need to phase the project?

56. East Cardio Room 118 Statement of Work - Task 1 Replace all louvers with aluminum frame awning windows on the east side of the room.

a. Please provide the dimensions for the existing louver rough opening to include the thickness of the wall for both the east and west wall?

See revised SOW. Wall thickness is approximately 8”.

b. Does the facility require only one awning window per louver opening on the east wall?

Each square will be an awning window, total of 9. See revised SOW.

57. East Cardio Room 118 Statement of Work - Task 3 Replace 9 each existing duplex receptacles along the walls and add 1 new receptacle.

a. Please provide an accurate drawing of the location and the correct number of receptacles?

We believe that the drawing shown as part of the scope of work is incorrect in respect to the numbers written in the statement of work. Drawing shows 10 existing and 3 new, statement of work says 9 existing and 1 new.

58. East Cardio Room 118 Statement of Work - Task 4 Install 10 new 20 amp dedicated circuit single receptacles recessed on the floor.

a. Please confirm that each receptacle will have only one plug in and will have its own 20 amp breaker? Reflected on the revised SOW

b. Please provide information of the existing electrical panel to assure that the panel has the capacity for the additional circuits being required? Please provide a drawing with the location of the existing electrical panels within the building in relation to the Room 118?

c. Please clarify that the statement “recessed on the floor” means that the concrete will have to be cut to allow the cover plate of the receptacle be flush with the finish concrete floor or does it mean that the receptacle cover needs to be flush with the newly installed padded polyurethane athletic flooring that is mention in Task 2?

59. East Cardio Room 118 Statement of Work - Task 5 Remove all existing TV monitors receptacles.

a. Please confirm that not only the electrical receptacles for the TV monitor but also the signal outlets need to be removed by the contractor on the west wall.

No signal outlets will be removed by user.

b. Can the contractor extend the existing conduit /circuit to the adjacent wall to satisfy the installation of 4 each new 20 amp duplex receptacle for new TV? If not will the contractor be responsible to remove the existing conduit and wiring all the way back to the panel?

Remove all existing conduit and wiring all the way back to the panel. New conduit and wiring will run up inside the ceiling space to a JB and to the receptacles.

c. Will the contractor be responsible to provide a signal outlet for the TV on the new wall?

If so where is the originating point and will the contractor be responsible for the wire or just provide a pull string?

Users are responsible for the cables.

d. Who will be responsible for the removal and installation of TV’s from the wall?

The User.

60. East Cardio Room 118 Statement of Work - Task 7 Repaint the entire interior with the same scheme and color including trusses.

a. Please provide the paint color for the existing walls of the room as well as the trusses above? What is the type of finish for the paint? ie: flat, satin, semi-gloss.

Semi-gloss, Latex. Color will be provided later during pre-con.

b. Presently room 118 has an acoustical drop ceiling and therefore during the walk thru the trusses and the area above the ceiling could not be viewed. Does the facility still want to paint the area above the ceiling as removal of the ceiling grid and light fixtures will probably need to be removed to access and paint the above area?

Only paint below the ceiling grid.

c. Will the paint for the trusses be oil based and for the room walls latex?

All paint shall be latex.

61. East Cardio Room 118 Statement of Work - Task 9 Remove & Replace 3 each 36,000 BTU AC system to include piping, wirings and 30 amp disconnects.

a. Will the existing AC units need to be operational during the installation of the new AC system?

No

b. Please provide information on the location, name plate rating, and panel schedule for the electrical panel that the new circuit will be coming from.

Use existing circuit and panel if not already supplied from GM2 panel.

c. During the walk thru it was mentioned that 36,000 BTU was incorrect. Please provide the correct size of the new AC or the name plate information of the existing AC if the intent is to replace with the same size?

The correct size of the existing AC is 30,000 BTU. Replacing with 36,000 BTU.

Recommended Model is reflected on the revised SOW.

62. Scope of work for the Cardio Room 118 item 7 states to paint entire exterior. Please clarify the scope of work.

63. In the Cardio Room 118 is the Contractor responsible for providing new TV’s and associated mounts. If Contractor is providing TV’s, please provide details on what is to be installed.

64. In the Cardio Room 118 and 121 please clarify if there is a specific thickness we need to meet for the mirrors.

1/4” minimum thickness.

65. East Cardio Rm 118 states paint exterior but not interior as 121 indicates. Is the intent for interior paint only?

66. East Cardio Rm 118 states replacing ceiling tile, please provide a spec or manufacturer.

67. East Cardio Rm 118 states remove and replace the 3 AC units. Is the intent just to move the existing units to a new location?

Complete removal and replacement, See revised SOW.

68. Statement of Work, Item #1 – replace all louvers with aluminum frame awning windows on east side of the room. Please provide the dimensions of existing louver openings and quantities.

Total awning windows is 69. (Room 118 – 9 awning windows see page 10 of SOW & Room 121 – 60 awning windows see page 5 of SOW). Contractor to field verify. Follow window at the weight room. See attached window specification. (Arcadia or approved equal) No ATFP or Blast Proof, etc.

69. Statement of Work, Item #2 – What is the substrate of the existing raised flooring? Is it a concrete slab on grade or wood framing? Also request for length, width and area of this existing raised flooring to be removed.

Concrete slab ongrade. Approximately 319sqft. Contractor to field verify measurement.

70. Statement of Work, Item #6 – Sectional wall mirrors, please provide additional site photos for the existing condition of all mirrors to be replaced at all sides as applicable at Room All mirrors are to be replaced as stated in the revised SOW.

71. Statement of Work, Item #7 – There is not enough information for a painting subcontractor to figure out to repaint the entire exterior with the same scheme and color for this room/building, please provide additional photos and approximate dimensions showing the exterior elevations to be repaint. Painting of trusses is included in this statement of work item #7, however photos as shown indicated a closed ceiling with acoustical ceiling tile on grid, please clarify.

There will be no exterior painting for Room 121 except for where existing louvers are being removed and partially enclosed by new split face CMU. Paint color of split face CMU to match existing exterior color. No exterior painting for room 118, except for where the existing louvers are being removed and enclosed with drywall material inside and EIFS material outside. This is where the inside AC units will be installed. Paint for exterior to match existing exterior color. See revised SOW.

72. Reference Sheet 7, 2.

Has an Asbestos / Lead Paint survey been completed regarding the flooring under the raised platform in room 118?

SOW repaint the entire exterior with the same scheme and color including trusses. Exterior surfaces shall be prepared, prime, and painted w/2-coats.

75. Reference Sheet 7, 1.

Currently the exterior wall where AC units are located, the exterior is boarded up with plywood and painted to match existing. Do we remove all and replace with Masonite?

Remove all louvers and plywoods and replace with gypboard on the inside and Exterior insulation finishing system (EIFS) on the outside.

76. Reference Sheet 7, 6.

Please identify soffit line location where the new mirrors are to be installed above.

Above the wainscot line. Reflected on the revised SOW.

77. Reference Sheet 7, 6.

Will the mirrors at Rm. 118 have the same metal trim as Rm.121?

Yes. Reflected on the revised SOW.

78. Reference Sheet 7, 4.

There currently is not enough space in the existing panel to support the additional 10 dedicated 20amp receptacles.

A new sub-panel will need to be installed.

MG2 is 90% empty and can be utilized.

No new sub-panel will need to be installed. Electrical room adjacent to Room 118, the panel MG2 is 90% empty and can be utilized.

ROOM 121

82. West Cardio Room 121 Statement of Work -Task 1 Remove & Replace existing louvers with new awning windows:

a. What are the rough opening dimensions of the existing louvers?

See revised SOW for approximate rough opening dimensions. Contractor to field verify.

b. What is the material of the wall that the louvers are in. ie: concrete/CMU/etc?

Concrete.

c. What is the thickness of the existing wall?

d. One picture of the louvers show that there will be 6 awning windows in the upper existing louver opening and two awning windows in the lower existing louver opening. Please confirm that this is the correct number of new awning windows wanted?

83. West Cardio Room 121 Statement of Work - Task 3 Replace 8 existing duplex receptacles along the walls and add 6 each new receptacles.

a. The receptacle layout drawing of Room 121 shows 10 existing receptacles with its locations along the wall which is incorrect. The existing number that we counted is 6 and the location of the receptacles shown on the drawing is incorrect as well. Please provide an accurate drawing of the location and the correct number of receptacles.

b. The SOW says to replace 8 since the existing number is only six do we just replace only the six existing receptacles.

a. Please provide the paint color for the existing walls of the room as well as the trusses above? What is the type of finish for the paint? ie: flat, satin, semi-gloss.

Repaint the entire interior with latex/semi-gloss, the same color as trusses it will be discussed at PreCon. Exterior surfaces of new CMU shall be prepare, prime, and paint with 2 coats of the same color and scheme as the existing.

b. Will the paint for the trusses be oil based and for the room walls latex?

All paint shall be latex/semi-gloss.

86. West Cardio Room 121 Statement of Work - Task 7 large ceiling fans.

a. Please provide information on the panel to include but not limited to panel name, all existing circuits and panel schedule that this new circuit will be installed in?

Existing Panel “E” (30 space single phase 3 wire 200 Amps) in Room 121 only has 11 unused spaces available. Where would you suggest locating the additional circuits? We suggest upgrading the Panel E to a 3 – Phase 42 space panel to provide the required additional required circuits.

COURT AREA

97. Court Area ‐ Item 3 ‐ drain covers shall be 6" x 12" sections ‐ the existing is 6" x 24" sections. What is required? See Revised SOW

a. The present drain covers are stainless steel. Will the new replacement covers need to be stainless steel? NO

Please provide a decision as to replace the existing FPEW panels or add the aftermarket breakers. By adding the aftermarket breakers there will be no warranty of the breaker or anything that is added to the panel.

OUTDOOR AREA

108. Outdoor Area Concrete – Page 11 of the SOW asks for concrete pads in 3 areas. Please confirm the size of the new pad outside of Room 118. During the walk through a change in size was indicated. If the new slab is intended to hold the existing shed it will need to be 9’0” wide (see attached sketch). Please confirm the size of this slab.

109. Outdoor Area Concrete – Page 11 of the SOW states that the concrete is to be 4” thick. Is a thickened edge perimeter required? We suggest a minimum of 6”. Is compacted fill required? We suggest 4”. Is 3,000 psi fine – pump mix – acceptable?

COMPANY ADMINISTRATIVE

111. Please confirm that we do not have to register the JV entity on the System for Award Management (SAM) database until after we submit our bid proposal on August 02, 2018. Incorrect, you must be registered and valid in SAM by the time proposals are received on August 14, 2018.

112. Please confirm that we do not need to submit with the proposal any size determination for the Joint Venture entity received from the Small Business Administration. SBA Hawaii states that any Joint Venture protocol would be at the discretion of the Contracting Officer(s) noted in the Solicitation. SBA does not review or approve Joint Ventures outside of the 8(a) Program, therefore, they are unable to provide us with a size determination for the Joint Venture entity. Correct, you do not need to submit any size determination.

113. Section 00 21 16, Para D, I – Technical states (Technical Printed Copies are not to include the company name). Please clarify. The Project Management Capability narrative generally would have the contractor’s name referenced throughout.

121. Per the RFP, offerors shall provide proposed rates for future Task Orders. Do we fill out pages 13 through 22 of the RFP and submit that as our proposed rates? Yes. Submit your proposed rates on pages 13 through 22 of the RFP where indicated and submit.

122. Is the 3-page RFP document that states the Work Order Number or Work Request Number available? Looked on fbo.gov and the document isn’t available. There are no work order number or work request number available. The only number referenced for this solicitation is W912CN-18-R-0012.

123. Will the "Site Visit Register" be posted soon? You mention at the site visit, possible Friday, but Monday for sure. Site Visit “Sign In” sheet has been distributed on July 17, 2018 via email to those who attended the site visit.

124. Please change the proposal due date to a minimum of 2 weeks after all RFIs have been responded to. This time is needed to get final material pricing, subcontractor quotes, and all the required breakdowns to submit the price proposal. Proposal due date has been revised from August 2, 2018 to August 14, 2018.

125. Please confirm that the contracting office has the ability to receive a hard copy of the proposal package by FedEx between the hours of 8AM and 5PM Hawaii Standard Time. The contracting office has the ability to receive a hard copy of the proposal package by FedEx between 9:00AM to 3:00PM HST (Office Core Hours).

126. “Amendment 1” from FBO contains Amendment 0001 and 0002. Amendment 0001 appears to not only change the date, but section 00 21 16 “instructions to proposer.”

From the 12-page document it is not clear what has been changed (i.e., no changes to text colors, highlighting, etc). Please indicate which changes are captured in this document.

For each amendment there is the SF30 (Changes Only) and the Conformed Solicitation is an updated solicitation to include all changes made.

2018 12:00 AM. Can you confirm the correct time is 12:00 AM? The correct response date and time is 14 August 2018 at 10:00 AM HST. Amendment 0002 corrected the response due time.

129. Will additional questions be accepted after 10:00 AM, 20 July 2018 due to the extension of the bid date of 14 August 2018? No additional questions will be entertained or accepted.

130. Per the "Work Schedule" tab of Attachment 1A, how are we to fill out Column B and C without an assigned Start Date? We would need an estimated Notice to Proceed Start Date to fill this out appropriately. Disregard Work Schedule tab. Remove for the Seed Project.

131. Per the "Materials" tab of Attachment 1A, would we only need to list the major components of the work or do we need to get specific and also list wood, nails, screws, caulking etc.? You may list the major components and combine miscellaneous components into one line on this tab. The total on the materials page should match the materials total on the summary page.

132. Per the "Equipment" tab of Attachment 1A, should we also list the unit? (e.g. hr, day, week, month....) Yes. Description of equipment should include the Unit of Measure for each line item listed on the “Equipment” tab.

133. Per the "Summary" tab of Attachment 1A, do we need to do a cost breakdown (from row 19 thru 31) for each subcontractor? If we are to fill out a summary for each subcontractor, we will not be able to achieve an accurate grand total project cost. May we suggest you provide us with a sample (mock proposal) of how you would want us to fill out the summary of proposed costs? For the subcontractor cost breakdown, we would like to see the major components to include labor, materials, equipment, other direct costs, profit, overhead, bonds, insurance, and applicable taxes for each subcontractor. You may use your own spreadsheet or format that includes the same content.

134. May we suggest you take a look at the attached cost summary breakdown we currently use for state projects? The attached excel file includes all the major components that are also included in your Summary Tab - and it is easier to understand. We have each subcontractor submit their own summary breakdown (attached pdf includes Subcontractor

1) and we submit their summary breakdown along with the excel file. You may use your own spreadsheet or format that includes the same content as Attachment 1A.

135. On page 6 of the RFP it states: “Hardware and paint are to be purchased from AbilityOne.”

Upon reviewing what AbilityOne has to offer it is quite limited. Is it safe to assume that we must purchase whatever is available from them and whatever is not may be purchased elsewhere? Yes.

BID GUARANTEE / BONDS

136. Please confirm that the only bid guarantee requirement is for the bid guarantee in the amount of the seed project. Please confirm that a bid guarantee for the overall contract (base year) is not required. The bid guarantee is ONLY for the seed project and is 20% of your proposal. There is not a need for a bid guarantee for the base/overall contract.

Bid Guarantee Amount – “The amount of the bid guarantee shall be in the amount of the seed project’s proposal”. On other MATOCs, the standard bid guarantee for the base contract is usually in the amount of the minimum guarantee. In the case of this Army

MATOC it would be $20,000. If a separate bid guarantee is required for the task order, it is usually 20% of the bid price or $3,000,000, whichever is less. Bid guarantee is ONLY for the seed project. There is not a need for a bid guarantee for the minimum guarantee.

The bid guarantee has been revised as follows: “The amount of the bid guarantee shall be 20% percent of the bid price of the seed project.

If bid guarantee is still required in the amount of the seed project’s proposal, will it be acceptable to state 100% of bid price instead of an actual dollar value. No. The bid guarantee has been revised as follows: “The amount of the bid guarantee shall be 20% percent of the bid price for the seed project.”

137. Bid Bonds – please advise of the bid bond requirements for each task order, upon award of the MATOC. Will a bid bond be required for each task order RFP? Bid Bonds will be required for each task order. Performance and Payment Bonds will be required for each task order awarded.

138. Performance and Payment Bonds –please advise of performance and payment bonds requirements, upon award of MATOC. Will the performance and payment bonds be based upon a blanket bond for each contract year or per task order basis. Bonds will be required on a per task order basis.

139. Looking for confirmation that the bid bond amount is to be 100% of the proposal, as opposed to 20% that is usually required for Federal Project bids. The bid guarantee has been revised as follows: “The amount of the bid guarantee shall be 20% percent of the bid price for the seed project.”

140. Bid Guarantee - can we furnish a bid guarantee that is more than the amount of the seed project (not the exact amount)? The bid guarantee has been revised as follows:

142. Regarding Volume III – Tab D: Bid Bond - 52.228-1 Bid Guarantee – c ) The amount of the bid guarantee shall be in the amount of the seed project’s proposal. In lieu of listing the standard percentage, or maximum dollar amount on the Bond, will the including the language “The amount of the bid guarantee shall be in the amount of the seed project’s proposal” be acceptable on the Bond? 143. Section 52.228.1 – Bid Guarantee: Bid guarantee states the bond should be in the amount of the seed project proposal? Will it be acceptable to issue a bond based on a percentage (eg. 10% of the bid amount).
