PHA engine notifies SM their PHA is due
SM completes the self-assessment portion of the PHA from any authorized environment
SM uploads supporting documentation to the
PHA
SM signs/submits their completed PHA self-assessment
RR is notified via their RR dashboard a specified SM’s self-assessment needs review
RR views then accepts assignment of a SM PHA from any authorized environment
RR reviews, compares, then reconciles PHA input then associates any pertinent supporting information to the PHA
When findings require action, including IMR deficiencies, RR notifies appropriate stakeholders
RR signs then submits the record review of the specified SM’s PHA
MHA Provider conducts a mandatory person-to-person MHA
MHA Provider is notified via their MHA Provider dashboard that a specified SM’s MHA needs review
MHA Provider views then accepts assignment of a SM PHA from any authorized environment
MHA Provider reviews SM MHA portion of the
PHA
MHA Provider documents recommended referrals per protocol then contacts appropriate stakeholder(s)
MHA Provider signs then submits the MHA summary of the specified SM’s MHA
PHA Provider is notified via their PHA Provider dashboard that a specified SM’s PHA needs review
PHA Provider accesses and accepts assignment of a SM PHA from any authorized environment
PHA Provider reviews SM PHA summary then associates any pertinent supporting information to the PHA
When in a DRHA time window, PHA Provider reviews applicable DRHA requirements
