This Request for Quotation (RFQ) document, issued by the West Virginia Office of Shared Administration (OSA) Purchasing Division on behalf of the Department of Health Facilities (DHF) at William R. Sharpe, Jr. Hospital, solicits bids for an open-end contract to supply Intercool P-323-50 glycol. The contract is intended to provide the facility with glycol on a continuing basis, with deliveries to be made to the hospital located at 936 Sharpe Hospital Road, Weston, WV 26452. The solicitation does not specify response deadlines, bidder conference dates, or site visit requirements in the specifications document. The contract will be awarded to the vendor offering the lowest total cost while meeting all mandatory specifications, including product quality standards, timely communication, and reliable delivery capabilities.
Pricing for this commodity contract is based on vendor cost plus a percentage markup as detailed on the Pricing Page (Exhibit A), with vendors required to provide all-inclusive delivery fees covering fuel, environmental and regulatory compliance, recapture, shipping, and hazardous materials handling. The contract price is subject to fluctuation based on the Consumer Price Index (CPI) throughout the contract term, with the awarded vendor responsible for submitting CPI documentation showing price changes from the original bid amount. The contract may be funded in whole or in part with federal funds, making it subject to federal funding requirements and addenda. Vendors must designate a primary contract manager available during normal business hours and complete the Pricing Page in full to avoid bid disqualification. The State of West Virginia's standard payment procedures will apply, and vendor default provisions allow for immediate contract cancellation if specifications or legal requirements are not met.
