About this file

This file is a USDA Form AD-1048 certification document for RFP 26O-0924, issued by the U.S. Department of Agriculture in conjunction with Mohave, an Arizona-based educational purchasing consortium serving approximately 488 public agencies statewide. The Request for Proposal seeks student information systems solutions for Mohave's member agencies, with contracts awarded to one or more offerors for an initial one-year term with four potential one-year extension options. The specifications provided are based on known acceptable manufacturers, processes, and brands; however, offerors may propose alternate solutions that meet the specified quality and performance characteristics, with Mohave serving as the final judge on acceptance of alternative solutions. Members currently eligible to participate in any awarded contract as of August 27, 2026, are identified in the RFP attachment, though additional members may join during the contract term and are responsible for determining whether their purchases would materially increase the originally solicited volume.

The certification form requires prospective lower-tier participants to verify that neither they nor their principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from federal participation. Prospective participants must also certify compliance with USDA civil rights regulations and policies prohibiting discrimination based on race, color, national origin, religion, sex, gender identity, sexual orientation, disability, age, marital status, family status, income from public assistance, political beliefs, or retaliation for prior civil rights activity. The form includes standard federal compliance language regarding the System for Award Management (SAM) database checks and requires participants to provide immediate written notice if any certification becomes erroneous, with violations potentially resulting in suspension or debarment.
