Addendum to 52.212-1 – INSTRUCTION TO OFFERORS
1. This Addendum prescribes the format for proposals and describes the approach for the development and presentation of proposal data and provides detailed information regarding the site visit. These instructions are designed to ensure the submission of information essential to the understanding and comprehensive evaluation of proposals. The instructions permit the inclusion of any additional data or information an offeror deems pertinent within the stated page limitations. Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government reserves the right to make an award based on initial proposals received, without discussion of such proposals.
(a) A pre-bid/pre-proposal conference (e.g., site visit) will be conducted at Wright-Patterson AFB, OH on 16 July 2010 at 9:00a.m. All interested parties shall meet in the parking lot outside the Hope Hotel, Building 823, Area A (located off base just prior to entering the security checkpoint at gate 12A – the AFMC Headquarters Complex Gate), Wright-Patterson AFB, OH 45433-5000. Please part at the far end of the parking lot closest to the street. Everyone should congregate into the Lobby of the Hope Hotel (Enter under the canopy labeled “Conference Center”). Base transportation will be provided from the Hope Hotel. At the conclusion of the Site Visit, you will be returned to this parking lot to retrieve your car. The purpose of this Site Visit will be to visit the facilities used during performance of the contract. All participants are required to assemble in the Conference center no later than 8:50a.m. Gate 12A is located directly off of SR 444. Please view the web site below the directions to Gate 12A. http://mapquest.com/directions, enter the address 4375 Chidlaw Rd., Wright-Patterson AFB, OH 45433.
(b) Submit the names of all attendees (not to exceed two (2) per company) to Shelley Deardorff via email at shelley.deardorff@wpafb.af.mil or via phone at 937-522-4525 prior to 4:00 pm on Wednesday 14 July 2010. This information must be provided in advance in order to ensure access to the military base/conference site and adequate seating for the conference attendees.
(c) Bidders/Offerors are requested to submit questions to the point of contact noted above not later than 4 p.m. on 14 July 2010. Information provided at this conference shall not qualify the terms and conditions of the solicitation and specifications. Terms of the solicitation and specifications remain unchanged unless the solicitation is amended in writing. If an amendment is issued, normal procedures relating to the acknowledgment and receipt of solicitation amendments shall apply.
(d) A record of the conference shall be made and furnished to all prospective bidders/offerors. The record should include minutes of the meeting, including questions (on a non-attribution basis) and answers. These items will be posted on www.fbo.gov, solicitation FA8601-10-R-0012.
2. To ensure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to submit all documentation/information detailed herein, including terms and conditions, representations and certifications, and technical requirements. Failure to submit all required documentation/information may result in a proposal being ineligible for award. In submitting a proposal, offerors are agreeing to the terms and conditions as written in the entire solicitation. The reference in FAR 5.212-1(e) encouraging submission of alternate proposals is hereby deleted for this solicitation and that subparagraph is marked “reserved”. The proposal shall consist of three (3) separate parts; Part I – Mission Capability Proposal, Part II – Past Performance Proposal, and Part III – Price Proposal. Offerors shall submit one complete original hardcopy proposal and three exact hardcopies as described below. All pages in excess of the stated limitations will be removed and disregarded during the evaluation. In addition, each Part shall be submitted electronically on separate CD-R type Compact Discs (CDs). (Note: CD-RW type CDs may not be used on Air Force computers and will not be accepted.)
Main Cover Letter: (Optional) Limit One Page. If a cover letter is provided for the offeror's entire proposal, it should include the offeror’s CAGE Code for their Central Contractor Registration (CCR).
Part I – Mission Capability Proposal: Limit 100 pages. See Proposal Content, below.
Part II – Past Performance Proposal: Limit 20 pages. See Proposal Content, below.
Part III – Price Proposal: No page limitation. See Proposal Content, below.
3. In accordance with the Performance Work Statement (PWS), paragraph 1.4, the offeror shall provide a Quality Control Plan that addresses accountability and ensures the Government receives quality workmanship. The offeror shall also provide a Safety Plan in accordance with PWS paragraph 1.9 that provides acceptable safety procedures to be used during this contract.
4, In accordance with the PWS, the offeror shall provide a subcontracting plan for evaluation required by FAR 19.702(a). Include all elements required by FAR 19.704 and DFARS 219.704. Provide separate targets, expressed as dollars and percentages of total contract value, in each of the applicable, authorized NASIC Group and a total target for small disadvantaged business (SDB) participation required by FAR 19.1202.4. For conversion of the SIC to the NASIC refer to http://www.census.gov/epcd/www/naicstab.htm. Targets will be incorporated into and made part of any resulting contract. Describe initiatives planned to ensure maximum efficient utilization of small business, HUBZone small business, small disadvantaged business, and women-owned small business concerns as subcontractors. For each applicable authorized NASIC Major Group, the offerors shall specifically identify Small Disadvantaged Business (SDB) concerns planned for use, the extent of commitment to those SDB concerns, and the complexity and variety of the work planed for those SDB concerns. If the offeror has an approved master subcontracting plan (FAR 19.704(b)), or an approved comprehensive subcontracting plan (DFARS 219.702), it shall submit an addendum with its proposal covering the additional information required by this solicitation. The addendum will be incorporated into and become part of any resulting contract, along with incorporation by reference of the master or comprehensive plan.4. It is anticipated that a preaward clearance will be required to ensure no contract is entered into with a person who has been found ineligible by the Deputy Assistant Secretary of Labor for reasons of noncompliance with the requirements of Executive Order 11246 concerning Equal Opportunity. In an effort to expedite the award process, all interested offerors are requested to verify that their companies have been found to be "In Compliance" with the Equal Employment Opportunity (EEO) regulations by the Office of Federal Contract Compliance Programs (OFCCP) office within the last two years. This can be verified by accessing the OFCCP National Preaward Registry at http://www.dol-esa.gov/preaward/. Failure to comply with this request will not disqualify an offeror from the competition.
5. The contracting officer has determined there is a high probability of adequate price competition in this acquisition and certification under FAR 15.406-2 will not be required.
6. Page Format: Each page of the proposal, other than foldouts, will be typed on 8-1/2” by 11” paper and will use Times New Roman, 10 point. Margins will be no smaller than 1”. Each page will be numbered. If printing appears on both sides of a sheet of paper, it is considered to be two (2) pages.

8. Proposals may be mailed, overnight mailed, hand-carried, or e-mailed to the above address so they are received by the contracting office by the due date and time. Due to their size, proposal will not be accepted via fax. All correspondence sent via e-mail shall contain a subject line that reads “FA8601-10-R-0012, Logistics Support Services.” E-mail filters at Wright-Patterson Air Force Base are designed to filter e-mails without subject lines or with suspicious subject lines or contents (i.e., .exe or .zip files). Therefore, if the prescribed subject line is not included, the e-mail may not get through the e-mail filters. Also .zip or .exe files are prohibited attachments and may be deleted by the e-mail filters at Wright-Patterson. Ensure only .pdf, .doc, or .xls documents are sent as attachments to e-mail. The e-mail filters may delete any other form of attachments.
9. Proposal Content:
a. VOLUME I – MISSION CAPABILITY PROPOSAL
(1) The proposal shall clearly demonstrate all mission capability/technical requirements stated in Performance Work Statement (PWS) and elsewhere in the solicitation to be eligible for award. The offeror should provide documentation which supports all the mission capability subfactors described below. The information provided should be adequate and sufficient to allow the Government to evaluate an offeror’s technical capability.
(2) As the technical approach will primarily describe and demonstrate the capability of the offeror to fulfill all requirements stated in the PWS, the mission capability proposal should be specific and complete in every detail. The proposal shall not merely offer to perform work in accordance with the PWS. It shall outline the actual method proposed as specifically as possible.
(3) It is the offeror’s responsibility to ensure the completeness of the mission capability proposal; therefore, the technical evaluation panel for the Government has been instructed to evaluate on the basis of the information provided. The Government shall not assume an offeror possesses any capability unless specified in its proposal. Understanding of the requirements of the PWS must be demonstrated by more than a reiteration of the PWS. Repeating the PWS without sufficient elaboration will not be acceptable. Statements such as “the offeror understands,” “can or will comply with all the requirements of the PWS,” “standard procedures will be employed,” etc., are insufficient.
(4) The specific topics to be addressed in Part I are as follows:
SUB-FACTOR 1: Base Supply: This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Base Supply Operations. Offeror shall, at a minimum:
1) The offeror shall demonstrate an understanding of the requirement by submitting a description of their approach to performing the Base Supply functions as outlined in the Performance Work Statement;
2) The offeror shall demonstrate an understanding of the requirement by submitting a brief description on how they intend to provide personnel that have all the qualifications, licenses and certifications to operate the Base Supply function as outlined in the Performance Work Statement;
3) The offeror shall provide a Base Supply organizational chart describing positions and a narrative of qualifications for each individual;
4) The offeror shall describe their approach to staffing a 24-hour per day, 7 day per week contingency operation in conjunction with normal day-to-day operations.
SUB-FACTOR 2: Transportation:
Element 2-1: Personal Property: This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Personal Property. At a minimum, identify the following:
1) The offeror shall demonstrate an understanding of the requirement by submitting a description of their approach to performing personal property shipment/warehouse inspections;
2) The offeror shall demonstrate an understanding of the requirement by submitting a brief description on how they intend to provide personnel that have all the qualifications, licenses and certifications to operate the Personal Property functions;
3) The offeror shall provide a Personal Property organizational chart describing positions and a narrative of qualifications for each individual Element 2-2: Freight Traffic: This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Freight Traffic. At a minimum, identify the following:
1) The offeror shall demonstrate an understanding of the requirement by submitting a brief description on how they intend to provide personnel that have all the qualifications, licenses and certifications to operate the Freight Traffic functions;
2) The offeror shall provide a Freight Traffic organizational chart describing positions and a narrative of qualifications for each individual
3) The offeror shall describe their approach to staffing a 24-hour per day, 7 day per week contingency operation in conjunction with normal day-to-day operations.
Element 2-3: Combat Readiness: This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Combat Readiness. At a minimum, identify the following:
1) The offeror shall demonstrate an understanding of the requirement by submitting a brief description on how they intend to provide personnel that have all the qualifications, licenses and certifications to operate the Combat Readiness functions;
2) The offeror shall provide a Combat Readiness organizational chart describing positions and a narrative of qualifications for each individual
3) The offeror shall demonstrate their understanding of the Air Force Mobility process.
4) The offeror shall describe their approach to staffing a 24-hour per day, 7 day per week contingency operation in conjunction with normal day-to-day operations.
5) Explain their approach to tracking the Deployment Sequence of Events (DSOC) to ensure on-time departures.
Element 2-4: Military Passenger Terminal: This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Military Passenger Terminal. At a minimum, identify the following:
1) The offeror shall demonstrate an understanding of the requirement by submitting a brief description on how they intend to provide personnel that have all the qualifications, licenses and certifications to operate the Military Passenger Terminal functions;
2) The offeror shall provide a Military Passenger Terminal organizational chart describing positions and a narrative of qualifications for each individual
Element 2-5: Commercial Travel Office: : This offeror shall provide procedures, methods and plans for accomplishing effective and efficient Commercial Travel Office functions as outlined in the Performance Work Statement SUB-FACTOR 3: Transient and Aerospace Ground Equipment (AGE) Maintenance:
Element 3-1: Managing Multiple Concurrent Events:
1) The offeror shall provide procedures and methods to manage multiple events, to include:
-- Normal transient traffic, -- The Dayton Air Show, -- Off station Presidential Support.
2) The offeror shall demonstrate an understanding of the requirement by describing their approach to scheduling methodologies; mix of management, quality control personnel, and support personnel, technicians to include skill types and levels; and equipment.
Element 3-2: Maintenance and Utilization of Powered and Non-Powered Aerospace Ground Equipment (AGE):
1) The offeror shall provide procedures and methods to manage and utilize the AGE facility and equipment
2) The offeror shall demonstrate an understanding of the requirement by including in their approach the appropriate mix of management, quality control personnel, support personnel, and technicians to include skill types and levels.
Element 3-3: Hazardous Materials (HAZMAT) Program Management. Offeror shall provide their approach, at a minimum:
1) The offeror shall provide procedures, methods and plans for managing the Transient Maintenance organization’s HAZMAT program IAW the PWS.
2) The offeror shall demonstrate an understanding of the requirement by including in their approach the appropriate mix of management, quality control, and technicians to include skills, levels, and certifications.
SUB-FACTOR 4: Precision Measurement Equipment Laboratory (PMEL):
Element 4-1: Lab certification by AFMETCAL. The offeror shall describe their plan for obtaining AFMET certification as required in the PWS.
Element 4-2: Transportable Field Calibration Unit (TFCU) and Jet Engine Test Cell Support (JETS). The offeror shall describe their method for performing calibration and repair of Test Measurement and Diagnostic Equipment (TMDE), using TFCU and JETS, for geographically separated units as specified in the PWS.
Element 4-3: Onsite Calibration at WPAFB.
1) The offeror shall describe their approach for performing onsite calibration and repair of TMDE for units located on WPAFB as specified in the PWS.
2) The offeror shall demonstrate an understanding of the requirement by including in their approach the appropriate mix of personnel, equipment, supplies and vehicles to perform this service.
Element 4-4: Production.
1) The offeror shall describe their your approach to performing calibration and repair of TMDE supported by the WPAFB PMEL in accordance with PWS Section I, Subsection 4.
2) The offeror shall demonstrate an understanding of the requirement by including in their approach the appropriate mix of personnel, equipment, supplies and vehicles to perform this service.
b. VOLUME II – PAST PERFORMANCE PROPOSAL
(1) Offerors shall provide the past performance questionnaire (attached to the solicitation) to their references. The offerors shall instruct their references to complete the questionnaire and submit DIRECTLY to the point of contact identified in paragraph 7.b.(6)(a) by the due date identified in paragraph 7.b.(6)(b). Questionnaires received directly from the offeror, subcontractor, or teaming contractor will not be accepted. Questionnaires that are routed through the offeror, subcontractor, or teaming contractor from their references will not be accepted.
(2) Offerors shall submit information on contracts they consider relevant in demonstrating their ability to perform all requirements of the PWS. This information may include data on efforts performed by other divisions, corporate management, and critical subcontractors or teaming contractors, if such resources will significantly influence the performance of the PWS requirements. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. If this is a first time joint effort, each party to the arrangement must provide a list of past and present relevant contracts and provide questionnaires to their references. Past performance information pertaining to a subcontractor cannot be disclosed to the prime offeror without the subcontractor’s consent. Provide with the proposal a letter from all subcontractors that will perform major or critical aspects of the requirement, consenting to the release of their past performance information to the prime contractor.
(3) Offerors with vast numbers of relevant contracts to select from shall submit no more than six (6) relevant contracts/jobs performed presently or within the past five (5) years. Offerors are required to explain what aspects of each contract are deemed relevant to the proposed effort. Offerors are permitted, but not required, to submit information on significant achievements or explain past problems considered relevant to the PWS requirements.
(4). In addition to the data provided by each offeror in this part of the proposal, the Government may use data obtained from other sources (Past Performance Information Retrieval System, other Government sources, commercial sources, etc.) in the development of the Integrated Past Performance Assessment rating.
(5) For all current or past contracts deemed relevant, provide all the information requested in the following paragraphs:
(a) Administrative Data
1. Company/Division Name
2. Program Title
3. Contracting Agency
4. Contract Number
5. Type of Contract
6. Dates Performed
7. Contract Value (including any option years)
8. Name, Address & Telephone/Fax Number of Program Director/Manager, Administrative Contracting Officer (ACO) or CO.
9. Relevancy Data: Indicate the percentage of service actually performed by the offeror (prime), subcontractor (if applicable), and teaming contractors (if applicable). This information indicates the volume of required service actually performed by the offeror and company(ies) other than the offeror on past or current contracts in relation to Relevancy. When applicable, provide the name of the company for each subcontractor or teaming contractor. The offeror may provide clarifying comments in relation to Relevancy.
(6) Past Performance Questionnaire – Offerors shall distribute the Past Performance Questionnaire to all references provided in the Past Performance Proposal. Questionnaires may also be provided to the references of subcontractor or teaming contractors for submission directly to the contracting office when the offeror determines the submission will support the offeror’s Relevancy data.
(a) Instruct all references to complete the questionnaires and return them directly to Shelley Deardorff at 88CONS/PKBB, 1940 Allbrook Drive, Room 109, Wright-Patterson AFB OH 45433. Questionnaires may also be faxed or e-mailed. Fax number is 937-257-3926 (must have "ATTN: Shelley Deardorff). E-mail address is shelley.deardorff@wpafb.af.mil.
(b) Offerors shall ensure completed questionnaires are received not later than 4:00 P.M. (EST), 9 August 2010. Offeror's and their references may verify receipt by contacting Shelley Deardorff by email at shelley.deardorff@wpafb.af.mil or by phone (937) 522-4524.
(7) If no past performance information is submitted as a part of the offeror’s proposal, the entire proposal will be considered non-responsive and will not be considered for award.
(8) For those offeror’s who do not have any relevant past performance information to submit, a statement to that effect will satisfy the requirement for a past performance volume.
c. VOLUME III – PRICE PROPOSAL
(1) Pricing - Optional Services are identified in the Solicitation package. These services are identified with the word (OPTION) in the title. These CLINs will only be exercised if and when funding becomes available. The Government may determine that a proposal is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(2) General – All information relating to pricing data shall be included in this volume. Under no circumstances shall pricing data be included elsewhere in the proposal. Should such pricing data be included in the mission capability and/or past performance proposals, the result will be removal of such pricing data and all other information included on the same page with such data. This could result in the exclusion from evaluation of critical mission capability/technical and/or past performance information. The price proposal shall contain the following in the order shown:
(a) A cover letter stating that your proposal is good for no less than 90 days, a Point of Contact (name, phone number, fax number, mailing address and e-mail address), and your response to FAR 52.222-22.
(b) Pricing, Schedule B of the solicitation. Complete all blanks for unit and total prices. All dollar amounts shall be carried out two (2) decimal places.
(c) Completed FAR 52.212-3 (Offeror Representations and Certifications–Commercial Items (Mar 2005) (attachment 8)and DFARS 252.212-7000 (Offeror Representations and Certifications—Commercial Items (Jun 2005) (attachment 9).
(d) Completed provisions/clauses for Offeror’s Representations and Certifications (see 52.212-3). Offeror’s with completed and up-to-date representations and certifications on the ORCA website may state this instead of completing the Representations and Certifications.
(e) Required Statement Regarding Expiration Date of Proposal: A statement shall be provided that the proposal and prices will remain in effect for 90 days after solicitation closing date. Include this statement on the cover letter or last page of Pricing Schedule.
(f) Signed acknowledgement of any Amendment(s) issued against this solicitation (if applicable).
FAR 52.212-1 and Addendum thereto, FAR 52.212-2 and Addendum thereto and FAR 52.212-3 will be physically removed from any resultant award, but will be deemed to be incorporated by reference in any award resulting from this solicitation. NOTE: The contractor must maintain the minimum insurance required in FAR 28.302-7.
