DETERMINATION OF RESPONSIBILITY
1. In an effort to determine your company responsible, the following items are requested:
2. The names of at least three (3) suppliers and subcontractors that you do business with, to include points of contact and phone numbers.
3. A minimum of three (3) contracts (not more than 10), either ongoing or completed, government or commercial, to include points of contact and phone numbers.
4. A statement indicating how long you have been doing this type of work.
5. If your company is incorporated, when you became incorporated.
6. State whether or not any of the work will be subcontracted, and if so, state which parts.
7. State your company’s specialty.
8. State whether or not you will be renting any equipment; if so, state the item(s) and provide proof of ability to obtain rental (i.e., rental agreement).
9. State whether or not this work will conflict with any other ongoing government or commercial contracts you have.
