About this file

This is an Exhibit B Prime/Subcontractor Form for Volusia County, Florida's ITB No. 26-B-59IF related to Contract No. C-2075 for the El Portal Vehicular Beach Access Ramp reconstruction project. The form requires all bidders to list all proposed subcontractors and specify the dollar value and percentage of work assigned to each contractor (prime and subcontractors) across identified project disciplines, including demolition and hauling, site security fencing, excavation and grading, cofferdam and dewatering, sheet pile installation, concrete work, pavement painting and marking, signage, testing, and surveying. The total bid amount listed on this form must match the total bid price submitted on Exhibit G (Bid Price Sheet), and failure to complete and return this form may result in a non-responsive bid. The prime contractor is responsible for ensuring all subcontractors hold appropriate licenses, and any changes to subcontractors during project completion require prior written approval from the County Project Manager. The form will also be used to determine local preference in accordance with Section 2.8, D of the solicitation, though the project summary indicates no local preference provisions are applicable due to FEMA funding restrictions.

The form is designed to track contractor composition and subcontractor participation, with bidders required to complete only the green-shaded cells representing dollar values assigned to each contractor, while all other cells are automatically calculated. The total of all work assignments must equal 100 percent, as verified in the grey cell at the bottom of the form. Contractors may add additional subcontractor disciplines in blank rows if required for the project. The form serves as a mechanism for the County to monitor prime contractor and subcontractor involvement and will be used in conjunction with federal requirements mandating that contractors make affirmative efforts to contract with small and minority businesses, women's business enterprises, and labor surplus area firms, though no specific set-asides are designated for disadvantaged businesses or small enterprises under this solicitation.
