About this file

This is a Construction Bid Submission Review Form created by the State of West Virginia Department of Environmental Protection (DEP) on August 11, 2017. The document serves as an informational guide for potential bidders on construction projects and outlines common bidding requirements and problems that may result in bid disqualification. The form clarifies that it is not a complete list of requirements for any specific project and does not encompass every possible issue that could disqualify a bid. All prospective bidders are required to thoroughly review the Request for Quotation (RFQ), all supporting documents, and related instructions in their entirety to identify actual bidding requirements and specifications.

The document identifies two categories of disqualifying errors. Errors resulting in immediate bid disqualification include failure to attend mandatory pre-bid meetings, failure to sign required signature pages, failure to supply a valid bid bond, failure to meet mandatory RFQ requirements, failure to acknowledge addenda when required, late bid submissions, federal or state debarment or suspension, outstanding environmental enforcement actions, and unresolved DEP vendor complaints. Errors that may cause bid disqualification before contract award include outstanding debt to the state or political subdivision, workers' compensation or unemployment compensation delinquency, failure to obtain required bonds and insurance, failure to provide a West Virginia contractor's license number within one day of request, failure to supply a signed drug-free workplace affidavit within one day of request, and failure to provide a subcontractor listing within one business day of bid opening or DEP request.
