About this file
This document is a Contract Amendment issued by the Arizona Department of Economic Security for the Hunger Relief Program - The Emergency Food Assistance Program (TEFAP), with Community Food Bank Inc. as the contractor. The contract amendment (Amendment Number Fourteen) extends the contract term from July 1, 2024, through June 30, 2025, and introduces several modifications to the Special Terms and Conditions, including definitions of "vulnerable adult," updates to personnel record requirements, and new provisions for background checks and fingerprinting.
The amendment focuses on establishing comprehensive background check protocols for employees and volunteers providing direct services to children or vulnerable adults. It requires Central Registry Background Checks for contractors, subcontractors, and their employees, with specific guidelines for disqualification and exception processes. The contract mandates maintaining confidential files of background check results for five years after contract termination and ensures compliance with Arizona Revised Statutes regarding fingerprinting, criminal background checks, and certification requirements for personnel working with juveniles or vulnerable adults. The amendment demonstrates a strong emphasis on protecting vulnerable populations by implementing rigorous screening and employment standards.
