About this file
This is Amendment No. 5 to Agency Term Contract No. GC868, a rate increase amendment executed between the Florida Department of Environmental Protection (FDEP) and HLM Environmental Remediation, Inc. The contract provides petroleum contamination site response action services in Florida's South Region. The amendment was executed on May 7, 2018, with an effective rate change date of May 22, 2018. The contractor submitted detailed justification for rate increases, which the Department reviewed and approved in accordance with Contract Provision 11.B.
The amendment increases rates for 235 pay items representing approximately 4.76% to 5.00% of the pre-existing contracted rates, with all increases not exceeding the maximum 5% allowed under Contract Provision 11.B.ii. The rate changes apply only to new work assignments offered on or after the Rate Change Date; all pre-Rate Change Date work assignments continue to apply the contractor's prior rate schedule. The revised Rate Schedule (Attachment D, Version 2.2) encompasses a comprehensive range of services including site assessments, drilling and boring, well installation, sample collection and analysis, laboratory testing, soil source removal, petroleum storage tank removal and disposal, remediation system construction and operation, and professional engineering and geological services. The rate increases affect pay items across all service categories, with individual rate adjustments ranging from approximately $0.10 to $5,250.00 depending on the specific service. All other terms and conditions of the original contract remain in effect, and the amendment does not add, delete, or otherwise modify any line items in the contract.
