About this file
This document is Amendment No. 14 to Agency Term Contract No. GC868 between the Florida Department of Environmental Protection (FDEP) and HLM Environmental Remediation, Inc. The amendment was executed on May 10, 2023, by HLM Environmental Remediation, Inc. (FEID No. 461745742) and the State of Florida Department of Environmental Protection, with an effective rate change date of June 1, 2023. The contract provides petroleum contamination site response action services in the South Region of Florida.
Amendment No. 14 implements rate increases across the contract's pay items outlined in Attachment D (Rate Schedule V3.5), effective for new work assignments issued on or after the Rate Change Date of June 1, 2023. Rate increases range from approximately 4.76% to 8.00% across more than 350 pay items, including drilling and boring services, well installation and maintenance, soil and water sampling and analysis, equipment mobilization, excavation and disposal services, remediation system packages, and professional services such as engineering oversight and report preparation. The amendment does not modify line items or scope of work—only rates are adjusted. Work assignments issued prior to the Rate Change Date continue to use the prior Rate Schedule (V3.4). The contractor's maximum authorized rates for each pay item are specified in Table 1 and the full Rate Schedule V3.5. The amendment also clarifies that no new Purchase Orders or Task Assignments shall be issued after the renewal term, though existing assignments may continue for up to twelve months following renewal term expiration.
