About this file

This is Amendment No. 12 to DEP Agency Term Contract No. GC868, an indefinite delivery vehicle executed between the Florida Department of Environmental Protection (FDEP) and HLM Environmental Remediation, Inc. The original contract was executed on February 27, 2014, to provide petroleum contamination site response action services in the South Region of Florida. This amendment, effective May 25, 2022, modifies pricing only and does not add, delete, or otherwise modify any line items contained in the contract.

The amendment updates Attachment D (Rate Schedule V3.4) with revised pricing for 441 pay items across multiple service categories including office activities, field activities, mobilization, drilling and boring, well installation and abandonment, sample collection, laboratory analysis, soil source removal, petroleum storage tank removal, debris and waste disposal, resurfacing, in-situ injection, trenching, remedial action construction, pilot test packages, system operation and maintenance, reports, and personnel services. Rate increases range from approximately 5% to 15%, with most items increasing between 10% and 15%. The new rates apply only to work assignments offered on or after the Rate Change Date of May 25, 2022, while all pre-existing work assignments continue to apply the contractor's prior rate schedule (V3.3). All other contract terms and conditions remain in effect, with the amendment's provisions controlling in any inconsistency between the contract and this amendment.
