December 10, 2020 City of Plant City Purchasing Department 302 West Reynolds Street Plant City, Florida 33563

RE: Request for Qualifications – Library of Engineering Services for Continuing Contracts

Dear Evaluation and Recommendation Committee Members:

The CivilSurv team (Team) is pleased to submit our qualifications for the above-referenced request for qualifications (RFQ) to provide services for the City of Plant City. We have assembled a focused Team that is a perfect match for the scope of services in your RFQ by combining the strengths of proven performers for the City with our local experience. You can be confident that we understand budget challenges, regulatory uncertainties, and the desire to provide environmentally sound solutions. Our Team sincerely wants to continue to serve Plant City as needs and projects arise. We will always be there as your trusted ally for maintaining and anticipating your infrastructure and facilities’ needs.

CivilSurv Design Group, Inc. (CivilSurv) is headquartered in Lakeland, Florida at 2525 Drane Field Road, Suite 7, only minutes from Plant City, and has served the area since 1980. Our veteran staff of professional engineers, urban planners, and surveyors & mappers have provided a wide range of services for many of the region’s largest projects, yet we remain small enough that our principals remain actively involved with our clients. Innovative, Nimble, and Responsive are adjectives we believe best describe our Team and our leadership. We are specifically submitting on the General Engineering, Stormwater Engineering, Water/Waste Water Engineering, Subsurface Utility Engineering, and Registered Surveying & Mapping categories of the Library of Engineering Services solicitation.

As the Prime Consultant, CivilSurv will contract directly with the City and be responsible for the overall management of our Team. We will lead all engineering design projects, whether they are General Civil, Stormwater, or Water/Wastewater.

We will also provide Surveying & Mapping services in-house, with our headquarters being less than 10 minutes from the City.

We are one of the largest multi-disciplinary government-focused teams in the region. We have a long and successful history of service to the area and seek to renew that relationship through the successful completion of this project. CivilSurv has assembled a Team with not only our staff but other experts in the field to supplement staffing. We deeply appreciate the City’s commitment to supporting local firms. CivilSurv's Teaming partners are Madrid CPWG, AECOM, and Patel, Greene and Associates, all located in Central Florida with a full staff of geotechnical, water/wastewater plant engineering, and environmental science personnel.

The organization chart included in Tab A of this proposal illustrates that we have all of the multi-disciplinary capabilities identified in your Request for Qualifications. These and other seasoned professionals from Madrid CPWG, AECOM, and Patel, Greene and Associates form a Team with the depth of resources and experience necessary to perform difficult tasks with aggressive schedules. We have the staff experience needed to address any of your needs.

The CivilSurv Team has selected 17 examples of professional services assignments similar to the contract being solicited by Plant City. The specific examples parallel the requested experience and demonstrate our capabilities and experience.

The diversity of project assignments and the confidence in the CivilSurv Team, as illustrated by the long-term working relationship our subconsultants and many other local government clients, establish that our Team is supplied with top-quality, dependable, and focused professionals that are committed to serving the City for many years.

We are used to working with tight budgets and finding ways to help keep the project within the available funding. We have also had extensive FDOT training in plans preparation, are certified by FDOT in preparing Maintenance of Traffic Plans and CEI inspections, and fully prequalified by FDOT in planning, surveying, minor and major roadway design, traffic signals, and CEI. Our resumes include anything from small projects to multimillion dollar projects. With our primary clients being County or City governments, our depth of resources is extensive performing many various types of public sector projects. Mark Frederick, PE will serve as the Contract Manager and will also oversee all Engineering services. Mr. Frederick has 20 years of engineering experience including more than 10 years managing client contracts. J.N. “Jack” Breed is responsible for serving as the Principal-in-Charge for this contract. Mr.

Breed has 37 years of experience in surveying and is a professor for surveying at the University of Florida teaching a class twice a week via telecom from a nearby UF extension campus.

Our eleven licensed professional project managers work on all aspects of the projects from planning, surveying, design through construction close out. This experience includes performing construction take offs and preparing construction costs estimates to compare with project budgets to insure that available funding is in place for construction of the project. This also includes extensive experience in construction bidding, construction contracts, review of any construction claims, preparation of change orders, value engineering, and construction close out.

Our organizational chart and staffing matrix illustrate the responsibilities of the personnel to be assigned to the project along with the reporting relationships among them. Our high standards are maintained through a practical and effective Project Management Program. The QA / QC program’s objective is to provide Engineering products based on sound principles and practices reflecting the highest standards to ensure that work products conform to the scope and project documents and is void of errors and omissions. The program is founded on the following four key principles:

• Proper Project Approach: Quality is achieved by adequate planning, coordination, supervision, and technical direction; proper definition of job requirements and procedures; the use of appropriately skilled personnel; and by individuals performing work functions carefully and doing it right the first time.

• Constant Project Supervision: Quality is controlled through daily checking, reviewing and surveillance of the entire team’s work activities by designated Supervisors and Managers.

• Formal Quality Reviews: Quality is verified by having a manager or designee performing QC functions consisting of surveillance and auditing of the work and the procedures followed when performing the work.

• Active Senior Management Oversight: Quality is assured through independent reviews by qualified staff of the processes, procedures, documentation, supervision, technical directions and staffing associated with the project development.

Procedures outlined in our standard QA / QC program are purposefully general to afford sufficient flexibility so they can be applied to the wide range of deliverable documents furnished to clients, as well as be tailored to suit the particular needs of various Regulatory Agencies. The QC Reviewer is provided a complete package of background information with the work product. This QC review package typically includes the work product to be reviewed, the project scope, client directives/changes, project criteria, technical standards/codes/regulations, computations, alternatives analyses and selected alternative justification, and previous review comments. The culmination of the production process prior to any phase submittal will include at least one independent review. This review will be performed prior to submittal by a senior level Professional not directly involved in the day to day project production. All relevant comments/recommendations will be added to the products prior to the phase submittal. Please see individual resumes for specific project experience.
