Offeror GuidelinesOfferor shall provide the name, title, and contact number for a client, and description of services provided for that client, for any contract that was canceled or prematurely terminated in the past six years. If responding as a Prime vendor and Subcontractor partnership, Offeror must specify the company for the specific cancellation. Offeror shall include details on the reason for the cancellation and the Offeror’s position relevant to the cancellation, including the final resolution and any lawsuits or settlements of the contract cancellation. Offeror shall add additional rows as necessary to provide all canceled contracts within the required timeframe.
Offeror shall provide contact information for individuals able to address questions concerning the cancellation. Failure to fully disclose a canceled contract within the scope of this requirement may result in disqualification. If the failure to disclose is not learned until a Contract has been awarded to the Contractor, the Contract may be terminated. In the event of such termination, the Contractor shall be liable for all costs associated with the re-procurement, including any increased costs for the services originally awarded.
