About this file
This is a Safety Data Sheet template for offerors to complete as part of their proposal submission for the P683 Aircraft Engine Repair Facility construction project at Naval Air Station Jacksonville, Florida. The form requires offerors to provide five years of historical safety data (2019-2023) including OSHA Days Away from Work, Restricted Duty, or Job Transfer (DART) rates and Total Recordable Case (TRC) rates reported as decimal figures. For any year where data is unavailable, offerors must affirmatively explain the reason. Additionally, offerors must explain any negative safety trends where DART rates exceed 3.0 or TRC rates exceed 4.5, including detailed descriptions of corrective actions taken to improve performance.
The form requires a Safety Narrative not to exceed two pages that addresses the offeror's Safety Management System (SMS) implementation and execution, covering management and leadership involvement, employee involvement, hazard prevention and control, worksite analysis, and safety and health training benchmarks. Offerors must also describe their process for evaluating and selecting subcontractors and detail their oversight mechanisms for ensuring subcontractor safety compliance throughout contract performance, including in-house workforce monitoring. For joint ventures or partnerships, a Safety Data Sheet must be provided for each contractor participant, though only one combined safety narrative is required. Insurance carrier information must be included. DART and TRC rates are not required from subcontractors.
