About this file
Attachment A is a Mandatory Features Checklist from the Brevard County Board of County Commissioners for a Group Dental Insurance solicitation effective January 1, 2026. The document outlines nine critical mandatory requirements that proposers must meet to be considered for the dental insurance contract, including submission of specific documentation, Florida licensing for at least five years, experience insuring groups with 4,000+ employees, and ability to provide dental DHMO and PPO plan options. Proposers must submit three hard copies of their proposal and one electronic copy, with a clear warning that any "No" response to the mandatory features will result in automatic disqualification.
The document specifies unique requirements such as having no minimum participation requirements, permitting retired employees and dependents to continue coverage, matching the County's definition of dependents (including children up to age 30), and maintaining a single contract with separate billing for each agency. The checklist requires the proposer to be the prime contractor performing at least 80% of contract work, with County permission needed for any subcontractors. Proposers must match existing in-force dental plan coverage and note any deviations or enhancements in their proposal. The document concludes with a authorization section for the proposing firm to certify the accuracy of their responses, requiring a signature, printed name/title, and date.
