About this file
Attachment A is a Mandatory Features Checklist for the Brevard County Board of County Commissioners' Group Dental Insurance procurement, effective January 1, 2026. The document outlines nine critical mandatory requirements that proposers must meet to be considered for the dental insurance contract, including submission of required purchasing documents, having a Florida license for at least five years, experience insuring groups with 4,000+ employees, and ability to provide dental DHMO and PPO plan options. Proposers must submit three hard copies and one electronic copy of their proposal, complete specific attachments in Microsoft Word and Excel, and meet detailed qualifications including no minimum participation requirements and coverage for retired employees and dependents up to age 30.
The document requires proposers to be the prime contractor performing at least 80% of contract work, maintain a single contract with separate agency billing, and provide benefits matching current in-force coverage limits. Critically, any "No" response to any mandatory feature will result in automatic disqualification from the procurement process. The checklist provides a signature section where the proposing firm must authorize the accuracy of their responses, indicating a formal commitment to meeting all specified requirements. While specific pricing is not detailed in this attachment, the document represents a critical screening mechanism to ensure only qualified dental insurance providers can proceed in the Brevard County procurement process.
