About this file

This is an Attestation Form (Attachment J) for RFP 26-87798 submitted to the State of Indiana. The form serves as a mandatory submission that confirms a respondent's compliance with all required proposal components, including the Executive Summary, Business Proposal (Attachment E), Technical Proposal (Attachment F), Cost Proposal (Attachment D), Artificial Intelligence Technical Proposal Questions, and Infrastructure Overview. Respondents must designate an Internal Response Manager as the dedicated State contact for solicitation-specific correspondence and provide their name, email address, and internal contact information. The form requires attestation that the respondent has read and understood confidential information protocols, contract terms and clauses, and the sample contract language, with options to accept the State's language or submit alternative language through Attachment E.

The Attestation Form includes several eligibility and preference claim sections requiring respondent certification. Respondents must affirmatively claim Buy Indiana certification status (with validation through the Buy IN Designation List as of the submission date) or confirm non-eligibility, and may claim Qualified State Agency Preference Scoring if applicable as a nonprofit agency for persons with severe disabilities under Indiana Code. The form requires submission of any confidential or redacted documents with specific filename formatting, identification of statutory APRA exemptions, and detailed rationales for each redaction. Additionally, respondents must provide proof of legal authorization for contract signature (corporate bylaws or board resolution), principal contact information including Company Bidder ID, FEIN, business type, and NAICS code, and confirmation of registration status with the Indiana Secretary of State.
