About this file
This is a Business Proposal Attachment E document from the State of Indiana (referenced through Indiana Department of Administration) that serves as a template for respondents to an RFP process. The document does not specify particular products or services requested, project quantities, response deadlines, or contract terms, as it functions as an instruction form rather than a detailed RFP specification. Instead, it outlines the required components that respondents must address in their business proposals, including company structure, financial documentation, references, and diversity subcontractor engagement. The form directs respondents to reference other sections of the primary RFP (particularly Section 1.24 for submission deadlines and Section 1.18 for Secretary of State contact information) for specific solicitation details, award timelines, and contract parameters.
The document requires respondents to demonstrate financial stability through Dunn & Bradstreet reports or audited financial statements for two fiscal years, obtain certification and registration with the Indiana Secretary of State prior to contract negotiations, and provide three client references with completed Attachment H forms submitted directly to idoareferences@idoa.in.gov by the proposal due date. Additional submission requirements include proof of legal authorization to bind the organization, documentation of engagement efforts with Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran-Owned Small Business (IVOSB) subcontractors, business continuity and disaster recovery plans, and information security protocols. For cloud-based systems, respondents must review and indicate acceptance or provide redlined edits with justification for Attachments B1, B2, and B3 covering Infrastructure-as-a-Service, Platform-as-a-Service, and Software-as-a-Service terms and conditions, with the State reserving the right to reject requested deviations from the standard Additional Terms and Conditions.
