About this file
This is a Cost Proposal Attachment for a Multi-Agency Cooperative (MAC) administrative services contract. The contract covers a two-year initial term spanning fiscal years 2026-2027 and 2027-2028, with an optional third year extension through 2028-2029. The scope of work encompasses five primary service components: Project Management, which includes personnel, resource management, kickoff activities, status meetings, and KPI oversight; Modernization, Tools Development, and Technical Implementation, supporting the MAC administrative framework redesign and piloting of time study tools and cost reporting templates with Area Agencies on Aging (AAAs); Operational Services covering daily MAC operations including claiming cycle execution, claim file generation, documentation maintenance, and stakeholder technical assistance; Evaluation, Monitoring, and Assessment activities for pilot evaluation, performance monitoring, and quality improvement tracking; and Transition and Knowledge Transfer supporting structured program handoff to the State at contract conclusion.
Respondents must populate all yellow-shaded cells with pricing for each component and both contract years, with all prices submitted as all-inclusive amounts with no additional costs accepted by the State. The Total Bid Amount will be automatically calculated as the sum of Year 1 (2026-2027) and Year 2 (2027-2028) costs and will serve as the basis for cost proposal evaluation. Respondents must include estimated costs for any additional activities not captured in the primary components that align with their technical proposals. For subcontractor commitments, respondents should calculate commitment percentages using the Total Bid Amount from the two-year initial contract period. Cost proposals must not include caveats or formatting alterations, as such modifications may result in disqualification.
