About this file

This is a Felony Conviction Notification form required by the State of Texas under Legislative Senate Bill No. 1, Section 44.034, applicable to Amarillo Independent School District (ISD) for the Food Service Smallwares contract opportunity. The form mandates that any person or business entity entering into a contract with the school district must provide advance written notice if the person, owner, or operator has been convicted of a felony, including a general description of the conduct resulting in the conviction. The notification requirement does not apply to publicly-held corporations. School districts retain the right to terminate contracts with entities that fail to provide required notice or that misrepresent information regarding felony convictions, though the district must compensate the contractor for services performed prior to termination.

The form requires an authorized agent to certify under penalty of perjury that the information provided is true and accurate. Signatories must select one of three options: confirming that the firm is a publicly-held corporation and thus exempt from the reporting requirement; certifying that the firm has no owners or operators with felony convictions; or disclosing the names of individuals with felony convictions and providing details of the conviction(s). This certification document is a mandatory submission requirement for all bidders responding to the Amarillo ISD Food Service Smallwares solicitation.
