About this file
This document is a Conflict of Interest Questionnaire form (CIQ) required by Texas Local Government Code Chapter 176 for vendors conducting business with the Amarillo Independent School District regarding RFP 3713-27.07 for Food Service Smallwares. The form must be completed and filed with the district's records administrator no later than the seventh business day after a vendor begins discussions or negotiations with the district, submits a proposal, or becomes aware of any conflicts of interest. The questionnaire requires vendors to disclose any employment or business relationships with local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, any gifts valued over $100 in aggregate to officers or their family members, and any family relationships with district officers.
The form establishes mandatory disclosure requirements under Texas law, with violations constituting a misdemeanor offense. Vendors must identify themselves, name any specific local government officer involved in a potential conflict, and provide detailed information about business relationships, income flows, and gift transfers. The questionnaire distinguishes between taxable income received by officers from the vendor and income received by the vendor from officers or their family members outside the governmental entity. Failure to file an accurate and complete questionnaire or knowingly providing false information may result in proposal disqualification or contract termination, as noted in the related RFP requirements.
