Based upon the plans and application, approved on April 12,2013, we are hereby issuing RSA 485-A:17 Alteration of Terrain Permit AoT-0542. As part of the processing of this application, DES granted approval to waiving specific requirements of Rule Env-V/q 1507.04, regarding Groundwater Recharge Requirements, Rule Env-Wq 1507.05 regarding Channel Protection Requirements, Rule Env-Wq 1507,06 regarding Peak Runoff Control, and Rule Env-V/q 1504,09(b)(2xb) regarding Site Specific Soil Mapping, with the finding that granting the waiver would not have an adverse impact on the environment, public health, public safety, or abutting properties, and that granting the request is consistent with the intent and purpose of the rule waived. Additional documentation relative to the waivers is contained within the fìle. This permit is subject to the following conditions:
L Activities shall not cause or contribute to any violations of the surface water quality standards established in Administrative Rule Env-Wq 1700.
You must submit revised plans for permit amendment prior to any changes in construction details or sequences. You must notify the Department in writing within ten days of a change in ownership.
3. You must notify the Department in writing prior to the start of construction and upon completion of construction, Forms are available at:

The approved plans, dated December I 9, 2012, and supporting documentation in the permit file are apart of this approval.
This permit expires on April 12,2018. No earth moving activities shall occur on the project after the expiration date unless the permit has been extended by the Department. If requesting an extension, the request must be received by the department before the permit expires. The Amendment Request form is available at:

This permit cloes not relieve the applicant fì'om the obligation to obtain other local, state or fècleral perrnits that may be required (e.g., from US EPA, US Army Corps of Engineers, etc.). Projects clisturbing over 1 acre may require a fèderal stormwater permit from EPA.

SYSTEM MODIFICATION APPROVAL
Re: PWS 1507010, DR 999757 – Transient, Non-Community Water System
Dolly Copp Campground, Martin’s Location, New Hampshire
Dear Mr. Dauer:
Our office hereby grants approval for the modifications to the above referenced project’s public water system. This approval allows for the or modification of the existing water system subject to compliance with Env-Ws 373, Design Standards for Non-Community Water Systems.
This approval is based on the submittal from HEB Engineers dated April 26, 2013, and subsequent correspondence, which describes improvements to the water system, including the replacement of all existing water mains and service connections.
In accordance with Env-Ws 373, Design Standards for Non-Community Water Systems, the following shall be submitted or confirmed as conditions of the final approval for the water system:
1. Upon completion of the project and prior to opening to the public, the owner shall contact surveyor Kevin Riel at (603) 271–2539 or kevin.riel@des.nh.gov to schedule an inspection.
2. Provision of a dedicated source raw water taps and flow meters, or the means to obtain discrete raw water samples and daily production volumes for each well source. Pump run-time hour meters are not required; however, we recommend these meters to aid in leak detection and to identify diminished source capacity in the future.
3. All non-potable/fire suppression water lines, such as the dump station, shall be labeled permanently, visibly, and clearly as NON-POTABLE.
4. Water system sampling shall be performed per the Master Sampling Schedule. The Master Sampling
Schedule and the sampling results are available at: http://www2.des.state.nh.us/DESOnestop.
5. The system construction shall meet Env-Ws 373.26. System Construction.
6. The water system Operation and Maintenance Manual and Final Record Drawings shall be made available onsite, upon request.
7. The water usage at this public water system shall not exceed the approved capacity of the wastewater disposal systems. If an increase in discharge or change in approved use is planned, please submit an amended plan the DES Subsurface Bureau for review and approval.
8. This approval does not relieve the applicant from the obligation to obtain other local, state or federal permits that may be required.

Dear Mr. Wagner:
Attaclred please fìnd Wetlands Permit number 2013-00655 to dredge and hll 7,246 square feet within wetlands and within the bed and banks of several unnamed intermittent and perennial streams to replace 33 existing culverts (ranging in size from 15" to 48" diameter), f:JrL745 square feet of wetlands for road shoulder widening, impact 600 square feet within the bed and banks of Cullrane Brook to replace an existing 23' span bridge with a new 25' span bridge, and temporarily impact 165 square feet of wetland to install a waterline. Work is being done to renovate the existing Dolly Copp Campglound within the White Mountain National Forest managed by the U.S. Department of Agriculture Forest Service. Work also includes replacing 10 existing culverts in uplancl areas, repaving existing site roads, and constructing a new RV campsite loop, The decision to approve this application was based on the following findings:
1. This project is classified as a Major Project per NH Administrative Rule Env-Wt 303.02(i), as stream irnpacts are greater than200linear feet within the bed and banks;
and per Rule Env-Wt 903.01(g)(1), as the project proposes the replacement of a tier 3 crossing,
2. The need for the proposed irnpacts has been demonstrated by the applicant per Rule Env-wt 302.01.
3. The applicant has provided evidence which demonstrates that this proposal is the alternative with the least adverse impact to areas and environments under the department's jurisdiction per Rule Env-V/t 302.03.
4. The applicant has dernonstrated by plan and example that each factor listed in Rule Env- Wt 302.04(a), Requirements for Application Evaluation, has been considered in the design ofthe project.
5. The project includes replacing an existing 23'bridge span (tier 3 stream crossing) within
Culhane Brook with a 25' bridge span.

6, The applicaut has submittecl the bridge crossing design as an Alternative l)esign Per Rule Env-'Wt 904.01).
7. The bridge replacement was submitted as an Alternative f)esign because it was founcl by the applicant that installing a bridge per the tier 3 clossing stanclards would not to be practicable due to overall costs, and because the existing bridge conveyed extrerne flows lì'om Tropical Storm h'ene witirout failure.
8, Only two of the 33 culvert replacements (culvelt 56 and S10) will be increased in size to accommoclate larger storm evenfs based on personnel site observations of overtopping during Tlopical Storm Irene.
9. The applicant has addressed the required Stream Crossing Rules of Chapter Env-Wt 900.
10. 'fhe NFI Natulal Iìeritage lJureau ("NIlB") commented that "It was determined that, although there was a NFIII recorcl (e.g., rare wildlife, plant, ancl/or natural comrnunity) present in the vicinity, we do not expect that it will be impacted by the proposed project."
11, DES has not received ally colrlrents for or opposed to the project.
12.In accotdance with RSA 482-A:8, DES finds that the requirements for a public hearing do not apply as the permitted project is not of substantial public inlerest, and will not have a signilÌcant impact on ol adversely affect the values of the palustrine and riverine resources, as iclentifìed under RSA 482-A:1. Also, the project will not irnpact wetland areas that are consideled to be of special value fì'om a local, legional, or state perspective pursuant to Rule Env-Wt 101.96.

Based upon review of the above referenced application, in accordance with RSA 482-A and RSA 485- A:77, a Wetlands Permit and Non-Site Specifìc Perrnit was issued. This permit shall not be considered valid unless signed as specified below.
PEIIMIT DBSCRIPTION: Dredge and fill 7,246 square feet within wetlands and within the bed and banks of several unnamed intermittent and perennial streams to replace 33 existing culverts (ranging in size fiom 15" to 48" diameter), fill 745 square feet of wetlands for road shoulder widening, impact 600 square feet within the bed and banks of Culhane Brook to replace an existing 23' span bridge with a new 25' span bridge, and temporarily impact 165 square feet of wetland to install a waterline. THIS APPROVAL IS SUBJBCT TO THE FOLLOWING PROJBCT SPECIFIC CONDITIONS:
1. All work shall be in accordance with plans by U.S. Department of Agriculture (USDA) Forest Service, White Mountain National Forest (WMNF) Engineering Departrnent dated December 19,2072, as received by the NH Department of Environmental Services (DES) on March 14,2013.
2. Any further alteration of areas on this property that are within the jurisdiction of the DES Wetlands Burean will require a new application and further permitting by the Bureau.
3. This permit is contingent on approval by the DES Alteration of Terrain Bureau.
4. This permit is contingent on approval by the DES Subsurface Systems Bureau.
5. Culvert outlets shall be protected in accordance with the DES Best Management Practices for Urban Stormwater Runoff Manual (January 1996) and the Stormwater Management and Erosion and Sediment Control Handbook for Urban and Developing Areas in New Harnpshire (August 1992).
6. In-stream work shall occur during low flow conditions in the growing season to minimize impacts to potential spawning areas of cold-water fisheries.
7. Discharge fi'om dewatering of work areas shall be to sediment basins that are: a) located in uplands; b) lined with hay bales or other acceptable sediment trapping liners; c) set back as far as possible from wetlands and surface waters, in all cases with a minimum of 20 feet of undisturbed vegetated buffer,
8. Dredgedmaterial shall beplacedoutsideofthejurisdictionoftheDESWetlandsBureau.
9. Proper headwalls shall be constructed within seven days of culvert installation.

1 1, All activities shall be in accordance with the Shoreland V/ater Quality Protection Act, RSA 483-8.
The owner is responsible for obtaining any Shoreland Pellnit that may be required per RSA 483-8, for oonstntction, excavation or fill that will occur within the Protected Shoreland.
12. Appropriate siltation/erosion/turbidity controls shall be in place prior to construction, shall be rnaintained during construction, and remain in place until the area is stabilized, Silt fence(s) rnust be removed once the area is stabilized.
1 3. Within thlee days of final grading, all exposed soil areas shall be stabilized by seeding ancl mulching clr"rring the growing seasotl, or if not within the growing season, by rnulching with tack or netting and pinning on slopes steeper than 3:1,
14. Where construction activities have been temporarily suspended within the growing season, all exposed soil areas shall be stabilized within 14 days by seeding ancl rnulching.
15. V/here construction activities have been ternporalily suspended outside the growing season, all exposed areas shall be stabilized within l4 days by mulching and tack. Slopes steeper than 3: 1 shall be stabilized by matting and pinning.
16. The contractor responsible for completion of the work shall utilize techniques described in the New Ilampshire Stormwater Manual, Volume 3, Erosion and Sedirnent Controls During Construction (December 2008).
GENERAL CONDITIONS TI{AT APPLY TO ALL DES WBTLANDS PBRMITS:
1 . A copy of this permit shall be posted on site during construction in a prominent location visible to inspecting personnel;
2. 'l'his pertnit does not convey a ploperty right, nor authorize any injury to property of others, nor invasion of rights of otl'rers;
3. The Wetlands Bureau shall be notiñed upon completion of work;
4. 1'his permit cloes not relieve the applicant liom the obligation to obtain other local, state or federal permits, and/or consult with other agencies as may be required (inclucling US EPA, US Army Corps of Engineers, NFI Department of Transportation, NI-I Division of Flistolical Resources (lrlH Depaltrnent of Cultural Resources), NI-IDES-Aheration of Terrain, etc.);
5. Transfèr of this perrnit to a new owner shall lequire notil'rcation to and approval by DES;
6. This permit shall not be extended beyond the current expiration date,
7. This project has been screened for potential irnpacts to known occurrences of rare species and exemplary natural communities in the immediate area. Since many areas have never been surveyed, or have received only cllrsory inventories, unidentilied sensitive species or comûtunities may be present.
