14. DESCRIPTION OF AMENDMENT/MODIFICATION Project Name and Location: CRESCENT DISTRICT OFFICE AND SITE
IMPROVEMENT PROJECT
This amendment addresses the following (see attached addendum)
1. Revisions to Specifications and Drawings (see attached)
2. Pre-Bid Inquiries (PBI’s) including Preproposal Meeting Notes and Requests for Information (RFI’s)
3. Preproposal Attendee Sign-In Sheet (see attached)
4. Hazmat Final Report and Soils Final Report (see attached)
5. Proposal due date is extended to May 8, 2014 4:30pm COB PST
By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

REVISIONS, PRE-BID INQUIRIES, REQUESTS FOR INFORMATION CRESCENT DISTRICT OFFICE AND SITE
IMPROVEMENT PROJECT
REVISIONS
REV-007
Specification section 01 10 00, 1.04.C. requires that a Phasing Plan be submitted for review and approval prior to site and building demolition. UPDATE: The Phasing Plans submitted by the Contractor shall consider similar measures for parking, access, utility service, accessibility, fencing, surfacing and grading as shown on the initial temporary parking/grading plan shown on sheet C301 of the contract drawings.
REV-008
There was an embedded font error in the title blocks on several of the Mechanical, Electrical and Plumbing drawings that prevented some characters from being printed properly. The font error has been fixed and the revised drawings are attached to this amendment. Remove the original Mechanic, Electrical and Plumbing drawings and replace with those attached. NOTE: there are
NO design changes included in these revised drawings, only font error corrections.
REV-009
Revise Detail 6/C604. Effluent filter changed from 4” to 8”. Remove the original sheet C604 and replace with the attached revised sheet C604. This item will be incorporated into the “Issued for Construction Set”.
REV-010
Revise Detail 1/C605. Stainless steel tee and elbow fittings changed from FL to FPT. Gate valve and ball valves changed from
Stainless steel to bronze. Check valves not required to be “lever and weight” style. Remove the original sheet C605 and replace with the attached revised sheet C605. These items will be incorporated into the “Issued for Construction Set”.
REV-011
Revise Detail 1/C607. Change support post material to 3/16” HSS. Clarify pipe supports. Add plate blocking to swing arm support bracket. Remove the original sheet C607 and replace with the attached revised sheet C607. These items will be incorporated into the “Issued for Construction Set”.
REV-012
Revise Drawing Sheet A-E105; delete panel ‘2SDP’ which is not needed. This item will be incorporated into the “Issued for
Construction Set”.
REV-013
Revise Specification 33 32 00 Wastewater pumping station – Part 2.6.A.1, Change valve body material from cast iron to bronze.
Remove the orignal specification 33 32 00 and replace with the revised attached specification. This item will be incorporated into the “Issued for Construction Set”.
Pre-Bid Inquiries (PBI’s) for Contract Bidding
PBI-002
If you are not anticipating the Government pulling permit(s) are you anticipating the county requiring permit(s)?
Per U.S. Code a Building Permit and Certificate of Occupancy are not required. However, the Contractor is still required to comply with the requirements of the Contract which include compliance with State and Local Codes. The Klamath County Building department will be providing building inspections as if a permit and Certificate of Occupancy are required. A courtesy Certificate of Occupancy may be issued by the County.
Other permits that may be required for this project include but are not limited to a Klamath County Public Works Permit for work done in County R/W (Main Street) and an ODOT permit for any work done in or adjacent to ODOT R/W. It is the responsibility of the Contractor to obtaining these permits and to pay for any inspection fees associated with the permits. No permit will be required from the Crescent Water District; however Contractor will be responsible for scheduling inspections with Crescent Water District.
PBI-003
Do you envision a Fire Alarm plan/review?
The Fire Alarm and Fire Sprinkler systems are design/build and deferred submittals are required. The submittals will be reviewed by the A/E and forwarded to the Authority Having Jurisdiction (AHJ), in this case, Klamath County for review and acceptance.

How many people are scoring the bids?
Four persons – Bids will be evaluated using an adjectival rating system. Award will be made from a determination of Best Value to the government and the contract format will be a Negotiated Firm Fixed Price (FFP)

Are Proposals to be submitted for bid by Subcontractors or Prime Contractors only?
Proposals are required to be submitted from Prime Contractors only.

Will copies of the Pre-Proposal meeting Sign-In sheet be attached to the next Solicitation Amendment?
Yes, see attached Pre-Proposal Sign-In sheet
PBI-007
Is it acceptable for bids to be submitted electronically via email?
No, may be mailed or hand delivered to the address specified in the solicitation.
PBI-008
Who is the LEED Commissioning agent? And is the contracting with the agent a requirement of the Contractor?
The LEED Commissioning Agent is Lew Seagraves – Lew has been contracted by the government.
PBI-009
Who’s responsibility is the HAZMAT Abatement? Government or Contractor?
HAZMAT abatement is the responsibility of the Contractor.
PBI-010
Was an underground survey done with the HAZMAT Study? What about Unforeseen Site/Conditions, i.e. underground asbestos pipe?
The HAZMAT site investigation was only performed on the structures of the site, no underground survey was performed as part of this investigation (See attached Report). If unforeseen Hazardous Materials are encountered the Contractor will be expected to notify the proper authorities and mitigate as required. Any abatement of Hazardous Materials that are not represented in Government provided documents or that are not reasonably ascertainable through site investigations may be covered by the Differing Site Conditions Clause, FAR 52.236-2 Differing Site Conditions

Is there a Bonding Estimate?
As stated in the solicitation the Budget Estimate is between $1,000,000 and $5,000,000
PBI-012
Is there any concern about a Certificate of Occupancy?
Per U.S. Code a Building Permit and Certificate of Occupancy are not required. However, the Contractor is still required to comply with the requirements of the Contract which include compliance with State and Local Codes. The Klamath County Building department will be providing building inspections as if a permit and Certificate of Occupancy are required. A courtesy Certificate of Occupancy may be issued by the County.

Is the County being used as an inspection Agency?
Yes, the Klamath County Building department will be providing building inspections as if a building permit and Certificate of Occupancy are required. The government has paid for all initial building department inspection fees. Any building department re-inspection fees will be the responsibility of the Contractor. The Contractor will also be required to obtain the necessary permits from Klamath County Public Works and pay any associated permit/inspection fees for work done within Klamath Co R/W.

Is there a provided Submittal List?
No, there is not a provided Submittal List.
PBI-015
Are there LEED FSC Goals? (i.e. Engineered Lumber)(% or Quantities)
The Forest Service is not pursuing the MRc7 Certified Wood Credit for the LEED certification so there is not a requirement of the Contractor to provide FSC certified wood.

Do the Specifications show LEED requirements/goals (% or #s)?
Yes, see Specifications 01 35 14.01, 01 35 14.02, 01 35 15 and 01 35 16 for LEED requirements.

Is there a Pre-Qualified list of Prime Contractors?
No, there is not a Pre-Qualified list of Prime Contractors.
PBI-018
Concerning Temporary Parking/Visitor Parking – Snow Removal is who’s responsibility?
The Government will retain responsibility of Snow Removal for temporary parking/visitor parking.
PBI-019
How about snow removal for the entire site?
The Contractor will be responsible for Snow Removal in Construction Areas that are outside of the areas described above in PBI-018 as necessary.

Are there Liquidated Damages in this Contract?
No, there are no Liquidated Damages in this contract.
PBI-021
What are the provisions/requirements for Winter Conditions (i.e. Concrete Blankets/Heating)?
The Contractor shall plan according for winter conditions in all aspects of construction. For Concrete Cold-weather placement and protection, comply with ACI 306R and contract specifications. It is up to the Contractor to verify and understand ALL cold weather requirements in the Contract Documents for this project. For interior and exterior products it is also required that the Contractor install per manufacturers recommendations, including meeting any installation and application temperature requirements.

Is there a RFI cut-off date? Who should RFI’s be directed to?
Currently there is no RFI cut-off date. RFI’s should be directed to either the CO or COR.
PBI-023
Is there an anticipated move in date for the Government to occupy the new Building?
There is not a set date for Government move in. Government will work with the Contractor. Currently the Project Completion Date is 9/30/2015 as outlined in F.2.
PBI-024
Minority and Female Participation goals?
Contractors are required to make good faith efforts to meet the established goals, the goals are not quotas and no sanctions are imposed solely for failure to meet them. Goals are neither quotas, set-asides, nor a device to achieve proportional representation or equal results. Rather, the goal-setting process is used to target and measure the effectiveness of affirmative action efforts to eradicate and prevent barriers to equal employment.

Certified Payroll will be required to be submitted with every payment application.

Are there any Badge or Security requirements for workers who are accessing the project site?
No, there are no badge or security requirements for accessing the site.

Will the Government be giving codes/cards for buildings & gates?
The Contractor will be given access to buildings/gates a necessary. Access will be restricted to normal working hours unless special arrangements are made with the C.O.R.

Who has ownership of Overhead power lines relocation concerning the Fire Bay?

Does Midstate Electric provide power at the site?
Yes, Midstate Electric Co-op is the power provider to the site.

Paint Striping of the Temporary Parking area?
This will be a one-time application per temporary parking area by the Contractor.

What is the status of the furthest building to the South at the site?
This is building # 1305. This building will be demolished in phase I.

Are there crawl spaces under the existing main office building?
The existing main office is a combination of original structures and add-ons. Some have crawl spaces some are on a slab.

Is the Govt. doing any “Owner” salvage of the existing structures?
No government salvage of existing structures is anticipated at this time.

Will all Government Property be removed prior to Demo?
Yes, for buildings requiring demolition the government will have all property removed prior to Contractor starting Demo.
At 90 days prior to new Administration Building completion contractor shall provide Government with tentative move in date. Once the new Administration Building is ready for occupancy, the Contractor shall plan on Government move operations taking up to 3 weeks.

What is the status of the box out front of the current office? (Septic System Plug Transclosure)
The current Transclosure will be removed and replaced with a new one, large enough for 4 plugs.

If the subs have questions RFI’s during bidding who do they submit them to?
All RFI’s should be directed to the Contracting Officer Representative or Contracting Officer

What is the groundwater situation at the site?
See attached Soils Report for groundwater and soil conditions at the site. Note that the Soils Report provides recommendations and it is up to the Contractor to follow the requirements of the Plans and Specifications. Bidders should also consider potential groundwater conditions and plan for appropriate construction measures if high groundwater is encountered during construction.

Will there be impacts to the Contractor due to increased activity during Fire Season?
It is not anticipated that there will be additional impacts to the Contractor due to Fire Season activities.

Can a bidder only respond with a bid for base items?
No, in order to be considered responsive, a bidder must provide a completed schedule of items, including bids on the 6 optional items.

Is there a material take-off that can be provided for this job?
No, there is not a material take-off that can be provided to bidders. It is up to the bidders to perform their own material take-offs.
Requests for Information (RFI’s) for Plans and Specifications
RFI-002
Is the base design for framing a conventional stud framing or a pre-engineered framing system?
The base project design for the Administration Building is intended to be standard framing. Pre-engineered panels will be considered as a substitution after award as long as the Contract Document requirements are followed.
