GENERAL INFORMATION

PBS Engineering and Environmental Inc. (PBS) has performed an asbestos and lead paint survey at the Crescent Ranger District Administration Site located at 136471 Highway 97, Crescent, Oregon. The survey was conducted in general accordance with OSHA regulations in 29 CFR 1910.1001, DEQ regulations in 340-248-0270, and the Deschutes National Forest Service Contract #: AG-04GG-C-12-0025. Based on the information gathered during the site inspection and laboratory analysis, this report contains the following information:
A summary of asbestos-containing building materials (ACBM) discovered during the inspection, including a material description, location of each identified ACBM, and the estimated quantity;
A summary of lead paint sampling, including sample descriptions, locations, and analytical results;
Sample inventories listing the sample number, location, material description, and laboratory results for each sample;
Laboratory analysis reports and chain of custody documentation (Appendix A);
Recommendations Inspector Certification (Appendix B)
SURVEY SCOPE
PBS endeavored to locate all accessible interior and exterior building components suspected to contain asbestos; no limited demolition was conducted to inspect areas concealed above ceilings, below floors, or within wall cavities due to the continued occupancy of the structures. Additional ACBM may exist in concealed locations in each structure. If concealed suspect ACBM are discovered during future demolition or renovation activities that are not identified in this report, testing should be performed prior to impact.
PBS also collected select paint chip samples from representative accessible major interior and exterior painted building components. No attempt was made to determine the paint history of the components that were sampled.

Note: Information provided to PBS by the Deschutes National Forest Service (DNFS) indicates that select building materials in Buildings # 1305, 1323 and 1325 previously tested positive for asbestos. These select building materials were not re-tested during PBS’ inspection; however, the results are conveyed in this report and the associated laboratory data is included in Appendix A.
Additionally, nine of the structures (Building # 1305, 1323, 1325, 2005, 2006, 2230, 2310, 2348 and 2602) were built prior to 1978 and are reported to have been re-painted (interior and exterior) with lead free paint; however, none of the underlying paint layers (if present) were previously tested for lead content.
CERTIFICATION
PBS has conducted a physical inspection of the structures listed above, compiled this report consistent with the survey scope, and certifies that the information is correct and accurate within the standards of professional quality and contractual obligations.

The following materials tested negative based on ASHARA sampling minimums and testing by an NVLAP participating laboratory. Although no asbestos was detected, it is possible that further sampling could indicate asbestos content.

All asbestos bulk samples were collected by an EPA AHERA accredited inspector and analyzed using Polarized Light Microscopy (PLM) with dispersion staining. Samples were submitted under chain of custody to Lab/Cor in Portland, Oregon (NVLAP # 200741-0) for analysis. The laboratory analysis report is presented in Appendix A of this report.
LEAD PAINT
A total of five (5) representative paint chip samples were collected from major painted components throughout the interior and exterior of the Fire Bunkhouse # 1305 and analyzed for lead content by Atomic Absorption Method.
Samples were submitted to NVL Laboratories, Inc. Seattle, WA (AIHA # 101861) for analysis. Laboratory results indicate that lead-containing paint was identified on the exterior siding, exterior window trim and select interior walls. The table below lists the location, material description, and laboratory results of the painted building components sampled during the survey.

(observed) 1Grey CAB applied with nails to the walls and ceiling of the Furnace/Water Heater Closet tested positive for asbestos. The CAB was observed to be in fair condition.
2Light grey fibrous felt underlayment tested positive for asbestos. The asbestos-containing felt underlayment is concealed under negative testing sheet floor covering (Type 1) and applied to wood subflooring with negative testing orange mastic. A second sample (020) of a miscellaneous flooring material collected in the Laundry Room Pantry/Water Heater Closet also identified the asbestos-containing felt underlayment. This material is presumed to be concealed under negative testing sheet floor covering (Type 1) throughout the Dining Room, Kitchen, Laundry Room, and Laundry Room Pantry/Water Heater Closet. The asbestos-containing felt underlayment is a highly friable material.
3Black tarry roofing mastic/sealant applied to the edges of the metal flashing around the base of the chimney tested positive for asbestos.

A total of three (3) representative paint chip samples were collected from major exterior painted building components and analyzed for lead content by Atomic Absorption Method. Samples were submitted to NVL Laboratories, Inc. Seattle, WA (AIHA # 101861) for analysis. Laboratory results indicate that no lead-containing paint was identified on the exterior siding and trim. No interior painted surfaces were identified. The table below lists the location, material description, and laboratory results of the painted building components sampled during the survey.

The interior painted surfaces were identified to be gypsum wallboard with spray-on texture and were not sampled. Note: Information provided to PBS by the Deschutes National Forest Service (DNFS) indicates that asbestos-containing floor tile and mastic was abated from this structure in 2008. It is possible that remnant asbestos-containing floor tile and mastic is present throughout this structure concealed by built in cabinets, fixtures, etc.
PBS presumes that the above listed asbestos-containing floor tile/mastic is a remnant material from the 2008 asbestos abatement project.

Samples were submitted under chain of custody to Lab/Cor in Portland, Oregon (NVLAP # 200741-0) for analysis. The laboratory analysis report is presented in Appendix A of this report.

A total of four (4) representative paint chip samples were collected from major exterior painted building components and analyzed for lead content by Atomic Absorption Method. Samples were submitted to NVL Laboratories, Inc. Seattle, WA (AIHA # 101861) for analysis. Laboratory results indicate that lead-containing paint was identified on the exterior siding and trim. The interior painted surfaces were identified to be gypsum wallboard with light texturing and were not sampled. The table below lists the location, material description, and laboratory results of the painted building components sampled during the survey.

1Reddish brown floor tile concealed under carpet and plywood tested positive for asbestos. The black adhesive mastic associated with the asbestos-containing floor tile tested negative for asbestos. The asbestos-containing floor tile and negative testing mastic is applied to a wood subfloor.
2Brown/Beige sheet floor covering previously tested positive for asbestos (15% Chrysotile). Information provided to PBS by the DNFS indicates that the asbestos-containing sheet flooring is concealed by newer (negative testing) sheet vinyl (Type 1), fiber board and plywood. The laboratory results for this material are included in Appendix A.
3Three samples of insulating putty (window glazing compound) applied along the exterior edges of window panes located on six perimeter windows tested positive for asbestos. The asbestos-containing window glazing compound was observed to be in fair to poor condition.

1Light green floor tile (Type 1) concealed under negative testing sheet flooring (Type 1 & 2) tested positive for asbestos. The black adhesive mastic associated with the asbestos-containing floor tile tested less than 1% asbestos. Under current regulations, the EPA, DEQ, and OSHA classify a material as asbestos-containing if analytical results indicate the material contains greater than 1% asbestos. The asbestos-containing floor tile and less than 1% black adhesive mastic is applied to a concrete subfloor.

wallboard tested negative in all three (3) samples. Based on these sample results, PBS recommends that all gypsum wallboard/joint compound wall and ceiling systems be treated as an asbestos-containing material, which may be rendered friable during demolition or renovation.
Note: The client may want to consider additional sampling of the gypsum wallboard/joint compound systems in an effort to better delineate the asbestos-containing wall and ceiling systems.
A second option is to consider additional analyses using a gravimetric/composite 400 point count method for the positive-testing joint compound/negative testing gypsum wallboard samples described above. It is possible that the additional analysis of this material may indicate a composite asbestos content which is below 1%. Composite analysis is only allowed for gypsum wallboard and joint compound system applications.
