About this file

This is an Affidavit of Eligibility Form for the Opportunity Zone Program administered by the Central Florida Tourism Oversight District in Florida. The program targets economic development and job creation in economically distressed communities across Orange, Osceola, Lake, Polk, and Seminole counties. Businesses seeking to participate in state and local contract opportunities, such as the 2027 Milling and Resurfacing Project, must certify that their main office is located within designated Qualified Opportunity Zones in one of the five eligible counties. The form requires businesses to provide their legal name, main office address, the specific opportunity zone county, and an authorized signature confirming eligibility.

The Opportunity Zone Program was established under the Federal Tax Cuts and Jobs Act of 2017 to encourage long-term capital investment in low-income census tracts. Award for associated contracts is determined by lowest bidder pricing on the combined base bid. Eligible businesses must complete and submit the Affidavit of Eligibility Form when responding to solicitations if they wish to claim program certification benefits. The program operates under federal guidelines administered through FloridaJobs.org, and businesses can verify their location eligibility through the Opportunity Zones Program mapping tool provided by that resource.
