About this file

This is a Scope of Work document for the State of Utah Division of Purchasing, in conjunction with the Department of Health and Human Services, outlining requirements for dental laboratory services. The RFP seeks an all-inclusive dental laboratory services provider capable of delivering both Traditional and Digital Impression Dentures on a per-item cost basis. The contract is anticipated to be awarded to a single responsive and responsible vendor with the highest evaluated score, though the state retains the option to award two separate contracts if one vendor specializes in Traditional Impression Dentures and another in Digital Impression Dentures. The contract term is five years with vendor pricing guaranteed for a minimum of one year.

The scope encompasses all necessary equipment and supplies required to deliver comprehensive dental laboratory services. Vendors must demonstrate capability in both traditional and digital impression denture production methodologies to be considered for single-contract award. Specific item quantities, response dates, due dates, site visits, and bidders' conference information would be detailed in the complete RFP documents. Pricing shall be structured on a per-item basis, and vendors must provide written guarantees that quoted prices will remain fixed for at least the initial contract year to ensure cost stability for the state.
