About this file

This Scope of Work document outlines dental laboratory services to be provided to the Utah Department of Health and Human Services (DHHS) under a contract anticipated to have a five-year term. The contractor shall provide comprehensive dental laboratory services on a per-item cost basis, including the fabrication of partials, dentures, night guards, orthotics, and splints. Specific deliverables include custom trays, wax rims, finishing work, repairs, and relines, with most items required to be completed within two weeks of receipt, except for general prosthetic and dental laboratory work which must be completed within ten business days. The contractor must maintain all required licenses, including a valid Business License, and provide annual documentation to DHHS. All materials and mailing containers necessary for prosthetic fabrication must be supplied by the contractor, who shall also provide itemized lists of charges with monthly invoices.

Quality standards are explicitly defined for both partials and dentures, requiring acrylic teeth, high-quality materials free of porosity or defects, proper fit to master casts, and correct specifications for all components. The contractor must remake any prosthetics that fail to meet contract standards at no additional cost to DHHS. Performance outcomes to be tracked include the percentage of prosthetic work orders completed within ten business days, the percentage of inmates receiving preventive dental services, and the rate of remakes due to contractor error. Billing requirements mandate monthly invoices containing the contractor's information, contract number, invoice number and date, and detailed service descriptions. Additionally, if the contractor processes sensitive data without existing FedRAMP or GovRAMP authorization, the product must be enrolled in the GovRAMP Progressing Snapshot Program within two months and achieve GovRAMP Core verified status within twelve months and GovRAMP Moderate Authorized status within twenty-four months of contract amendment.
