About this file
This is an Evaluation Criteria and Advancement document for Request for Proposal MK26-29 Dental Laboratory Services issued by the State of Utah Division of Purchasing in collaboration with the Department of Health and Human Services. The solicitation seeks all-inclusive dental laboratory services for the fabrication of both traditional impression dentures (using physical impression materials) and digital impression dentures (using intraoral scanner technology). The contract will be five years in length with vendors required to guarantee pricing for one year. The State anticipates awarding either a single contract to one vendor capable of providing both services or, at its discretion, two separate contracts to different vendors—one for traditional dentures and one for digital dentures.
The evaluation process employs a two-stage approach with multiple advancement thresholds. Vendors must first satisfy five mandatory minimum requirements on a pass/fail basis, including a minimum of five years' experience fabricating dental prosthetics, one year providing services to governmental or public health entities, the ability to supply all required materials, appropriate licensing and certifications, and compliance with GovRamp requirements. Qualifying vendors advance to technical criteria evaluation scored on a 0-5 scale across six categories covering prosthetics experience, government service experience, work with underserved populations, written protocols, high-volume production capacity, and scanner specifications. Vendors meeting the minimum technical threshold proceed to a 45-minute demonstration phase (30 minutes for product demonstration plus 15 minutes for questions), followed by the cost evaluation phase. Stage 2 evaluation, conducted only if the State elects not to award during Stage 1, mirrors the Stage 1 process and results in separate contracts for each denture fabrication method.

MK26-29
Dental Laboratory Services -Attachment E Mandatory Minimum Requirements & Technical Scoring Criteria
Evaluation Criteria & Advancement
Minimum Mandatory Requirements: Vendors must indicate "Yes" or "No" for each minimum mandatory requirement. To advance to the Technical Criteria Evaluation, the vendor and their proposed services must meet (answer "Yes" to) all mandatory requirements.
Technical Criteria Evaluation: Vendors must submit a detailed response to the technical criteria. An evaluation committee will score these responses on a 0–5 scale. To remain eligible for an award, vendors must meet or exceed the minimum technical score threshold outlined in the solicitation document.
Contract Structure & Evaluation Stages
Award Structure: This solicitation will result in two separate contracts: one for in-person dentures and one for electronic denture molds. The State reserves the right to:
· Award both contracts to a single firm.
· Split the awards between two different firms.
· Award a contract for only one of the services if deemed in the State's best interest.
Two-Stage Evaluation Process: The State will evaluate proposals in up to two stages:
· Stage 1 (Combined Services): The State will exclusively evaluate proposals from vendors offering both traditional impression dentures and digital impression dentures.
· Stage 2 (Individual Services): If the State elects not to issue an award upon the conclusion of Stage 1, the evaluation will proceed to Stage 2. This stage will evaluate vendors offering to provide only one of the services (either traditional impression dentures or digital impression dentures). Vendors not awarded in stage one will still be considered in stage 2 for whichever method(s) the evaluation committee scores high enough to make it to stage 2.
Definitions:
· Traditional Impression Dentures: A conventional denture fabrication process in which a dental professional uses physical, putty-like impression materials placed inside a specialized tray to capture a physical negative mold of the patient’s oral cavity.
· Digital Impression Dentures: A modern denture fabrication process that utilizes a handheld intraoral scanner to capture highly accurate, three-dimensional (3D) digital images of the patient’s oral cavity. This method entirely bypasses the use of physical impression materials and trays.
Minimum Mandatory Requirements (Pass/Fail, must pass all) 1-Vendor must have a minimum of five years of experience fabricating dental prosthetics, including full and partial dentures.
2- Vendor must have a minimum of one year of experience providing dental laboratory services to governmental or public health entities.
3- Vendor must be able to supply all materials required for the fabrication of dental prosthetics, including mailing containers.
4- Vendor must hold all required licenses, permits, and/or certifications to operate a dental laboratory and provide the services described in this solicitation.
5- Vendor must meet or agree to meet the GovRamp requirements.
Technical Criteria – Please submit a response to the technical criteria listed below 1-Please describe your company’s experience fabricating dental prosthetics, including dentures.
2- Please describe your company’s experience in providing dental laboratory services to governmental or public entities.
3- Please describe your company’s experience serving low-income, underserved, or special-needs populations.
4- Please describe the experience your company has in developing and following written protocols for prosthetic fabrication and case flows 5- Please demonstrate your company’s ability to produce high-volume dental prosthetics while meeting quality and turnaround time requirements.
6- Please provide a detailed overview of the scanner models your company offers or utilizes, along with their key features and technical specifications.
Demonstration Phase
· Prerequisites: To advance to this stage, vendors must meet all mandatory minimum requirements and achieve the minimum technical score threshold.
· Format: Invited vendors will have a total of 45 minutes for this phase, which includes a 30-minute demonstration of their proposed electronic solution, followed by a 15-minute Q&A session.
· Stage 2 Exception: If the State proceeds to Stage 2 and awards separate contracts for traditional impression dentures and digital impression dentures, the demonstration phase and its associated points will be waived for the vendor only providing traditional impression dentures.
Cost Phase
· Prerequisites: To advance to the Cost Phase, vendors must successfully meet all preceding thresholds: Mandatory Minimum Requirements, Minimum Technical Requirements, and the Minimum Demonstration Requirement
Stage 2
· Activation: Stage 2 will only be initiated if the State decides not to issue a single-vendor award during Stage 1.
· Process: The evaluation process in Stage 2 will mirror Stage 1.
· Award Structure: If activated, Stage 2 will result in contract awards to two separate vendors: one for traditional impression dentures and one for digital impression dentures.
