5.11 Service Contract Work Requests

Service Contract Work Requests (SCWR) are utilized to purchase replacement parts, equipment or incidentals that are required to meet the requirements of the contract, not otherwise stated in the contract. The Service Contract Work Request form, FTC-ADM-61 (Section D, Attachment 15) shall be utilized for this purpose. CLIN X009, as reflected in the pricing portion of this contract, shall have a fixed dollar amount for the year. The Contractor will be reimbursed for the exact amount of the paid invoice which shall be the same as or lower than the approved SCWR amount. The Contractor shall keep an SCWR log identifying in order each SCWR that has been submitted and the current fiscal year total dollar amount of items purchased and the remaining available amount of CLIN X009.

When the Contractor becomes aware of supplies not otherwise stated, a new piece of equipment that should be purchased, or other incidentals that may be needed throughout the year, the Contractor shall research and find three vendors who can furnish the supplies needed. The Contractor shall request a quote from each of the three vendors on the vendor’s letterhead for the price of the supplies that is required to be purchased and any other costs associated with the purchase and delivery of the item. When the three quotes are received, the Contractor shall attach those quotes in an email to the Uniform Issue COR, detailing what is required to be purchased, why the supplies are needed and from whom the Contractor recommends the supplies be purchased. When the COR receives the Contractor’s email, the COR will research and validate the vendor’s quotes to determine if they concur with the Contractor’s recommendation of who to purchase the supplies from. If the COR concurs, they initiate a SCWR of the supplies to be purchased, any other costs associated with the supplies, and their recommendation of who the supplies should be purchased from.

Once the SCWR is completed and signed by the COR, the COR will forward the SCWR to the CO for approval and price reasonableness determination. When the CO approves, signs and returns the completed SCWR to the COR, the COR will forward, in an email, the completed SCWR to the Contractor giving them permission to purchase the supplies and detailing which vendor to purchase the supplies from and other information that may be required when the item is received.

When the supplies are received, the Contractor shall notify the COR who will inspect and receive on behalf of the Government. No work shall be performed unless specifically authorized by the CO. Work that can be completed by the maintenance staff, but requires supplies to be purchased with an SCWR, shall be completed in accordance with CLIN X009 for material replacement of the pricing section of this contract and the ordering provisions of the contract.

Work that cannot be completed by the maintenance staff, but requires supplies to be purchased with an SCWR, shall be completed in accordance with CLIN X009 for material replacement.

Emergency oral requests shall be confirmed by issuance of a written SCWR within two working days. The COR will verify draft SCWR and vendor quotes before submitting the signed SCWR to the CO. If the CO agrees with the Contractors recommendation and reasons for purchase and if the CO finds pricing to be fair and reasonable, the SCWR process will be completed as usual. Copies of work orders, bids, etc. shall accompany the Contractor’s monthly invoices, charging the Government for the items and/or work. Cost for all equipment, parts, etc. that has been approved through the SCWR process shall be billed to the Government in arrears upon completion and acceptance by the COR on CLIN X009 on the Contractor’s invoice. Cost for maintenance work from an outside vendor that has been approved through the SCWR process shall also be billed to the Government in arrears upon completion and acceptance by the COR on CLIN X009 on the Contractor’s invoice.

NOTE: The Government reserves the right to furnish to the Contractor any or all parts and/or materials required for a particular repair as Government furnished property. If the Contractor furnishes the parts and/or materials through the SCWR process, the price to be paid shall be on the basis of established catalog or list prices, in effect when material is furnished, less all applicable discounts, but in no event shall such price be in excess of the supplier’s sale price to his most favored customers for the same item in like quality, or current price, whichever is lower.
