About this file
This is a Cost Proposal Form for Salt Lake City Corporation's Request for Proposals SLCI26057 for Third Party Fire Inspection Reporting Software. The solicitation seeks a contractor to provide a software system that collects, organizes, and transmits fire inspection data from independent inspection contractors to the Salt Lake City Fire Department in electronic format. The system must accommodate various inspection types including fire extinguishers, fire suppression systems, hood systems, fire alarm systems, emergency responder enhancement communications coverage systems, and fire watch reporting. The software should send reminders to building and business owners regarding inspection completion and submission requirements, and notify the Fire Marshal's office of non-compliant properties. The City seeks consistency with other Wasatch Front jurisdictions to minimize impact on building and business owners operating across multiple locations and jurisdictions. The initial contract term is five years.
Offerors must submit costs using the provided Cost Proposal Form with all pricing entered in designated yellow-highlighted cells. Evaluated costs consist of three components: implementation and annual maintenance costs (cell H25), cost per fire inspection (cell C28), and optional future enhancement hourly rates (cell D39), with the total proposed cost calculated in cell H42 and highlighted in green. The per-inspection charge cannot exceed $10 as established in Salt Lake City Code. All costs necessary to support fire inspections throughout the five-year contract lifecycle must be included on the form; any costs not provided will not be permitted in the resulting contract. Any deviation from the specified cost sheet format may result in proposal disqualification. Offerors may add additional cost lines for separate software modules or other applicable expenses as needed.
