About this file
This document is a Procurement Certification for Disclosure of Lobbying Activities form issued by the State of Florida Department of Transportation for use on federal-aid contracts. The certification is required for the City of Fort Myers' Architectural Services Continuing Contract procurement (RSQ 25-069), which seeks qualified architectural firms to provide professional services on an as-needed basis for various city projects. The continuing contract has an initial three-year term with two potential one-year renewal options. The City intends to select multiple qualified firms through a comprehensive evaluation process that includes initial scoring, potential presentations or interviews, and final ranking by a selection committee. Prospective firms must submit detailed qualifications by November 4, 2025, at 2:00 pm. The evaluation criteria allocate 45 points for the consultant's approach to the project and 55 points for experience and qualifications, with additional considerations for Minority Business Enterprise participation (13 points) and local vendor preference (5 points).
This certification form requires prospective participants to certify that no federal appropriated funds have been or will be paid to influence federal agency officers, Members of Congress, or congressional employees in connection with the contract award. If non-federal funds have been or will be paid for lobbying purposes, the prospective participant must submit Standard Form-LLL, "Disclosure of Lobbying Activities." The certification constitutes a material representation of fact required by Section 1352, Title 31, U.S. Code, with civil penalties ranging from $10,000 to $100,000 for failure to file required certifications. Prospective participants must also ensure that this certification language is included in all lower-tier subcontracts exceeding $100,000, with all subrecipients required to certify and disclose accordingly. The form requires execution by an authorized representative with signature, printed name, title, and date.
