About this file
This document is a Request for Proposal (RFP) No. BPM006719 issued by the Arizona Department of Administration's State Procurement Office for Statewide Emergency Response Services and Solutions. The document specifically contains Attachment 3, which addresses the Forced Labor of Ethnic Uyghurs Ban, requiring certification from contractors regarding labor practices in the People's Republic of China.
The attachment outlines compliance requirements under Arizona Revised Statute § 35-394, mandating that companies with ten or more full-time employees certify they do not use forced labor of ethnic Uyghurs or products/services produced by such labor. Contractors must select one of three options: affirm non-use of such labor, acknowledge participation in such labor, or qualify as an exempt contractor (sole proprietorship, fewer than 10 employees, or non-profit organization). In this specific document, USA UP STAR, located at 1760 Industrial Drive in Greenwood, Indiana, is the responding company, with MaryBeth Milici authorized to sign the certification.
